摘要:

目的分析重庆地区新生儿眼底疾病的发病情况及影响因素。方法对2016年1月至2019年11月在本院眼科使用Retcam3进行眼底病变筛查的38 970例新生儿的筛查结果进行统计学分析。将新生儿分为足月儿和早产儿两组,分析两组眼底病变检出率。新生儿视网膜出血及渗出样改变的影响因素采用单因素分析及Logistic多因素回归分析。早产儿按胎龄和体质量分组,研究各组早产儿视网膜病变(retinopathy of prematurity,ROP)的检出率。结果足月儿组、早产儿组各病种总体异常检出率差异具有统计学意义(P=0.014)。受检足月儿34 248例,共68 496只眼,眼底异常检出率为24.97%;其中视网膜出血检出率最高(8 131眼,11.871%),其次为视网膜渗出样改变(6 611眼,9.652%)。经Logistic多因素回归分析,性别、胎龄、分娩方式对新生儿视网膜出血的发生有显著影响(P<0.001),性别、胎龄、分娩方式、母亲妊娠糖尿病对新生儿视网膜渗出样改变的发生有显著影响(P<0.001)。受检早产儿4 722例,共9 444只眼,ROP检出率为8.13%;不同体质量和胎龄的早产儿,ROP检出率有明显差异(P<0.05)。结论重庆地区新生儿视网膜出血、新生儿视网膜渗出等检出率较高,其中性别、胎龄、出生体质量、分娩方式、母亲妊娠糖尿病为引起新生儿眼底病变的高危因素。

Abstract:

ObjectiveTo investigate the prevalence and risk factors of fundus oculi diseases in newborns in Chongqing. MethodsBetween January 2016 and November 2019, we screened 38 970 neonates for fundus diseases using wide-angle digital retinal imaging system (RetCam3) in our center, and compared the detections rates of fundus diseases between full-term and premature neonates. Univariate analysis and logistic multivariate regression analysis were performed to analyze the factors associated with retinal hemorrhage and exudative changes with undefined diagnosis. Subgroup analysis was carried out for the premature neonates to compare the detection rates of retinopathy of prematurity among the neonates with different gestational ages and birth weight. ResultsThe detection rates of neonatal fundus diseases differed significantly between the full-term newborns and the premature neonates (P=0.014). Of the 34 248 full-term neonates (68 496 eyes), 17 103 eyes had abnormal findings (24.97%), and the most frequent abnormality was retinal hemorrhage (RH), which was found in 8 131 eyes (11.871%), followed by exudative changes with undefined diagnosis (6 611 eyes, 9.652%). Logistic regression analysis indicated that a female gender, full-term birth and vaginal delivery were all associated increased risks of retinal hemorrhage (P<0.001); a male gender, full-term birth, vaginal delivery and maternal gestational diabetes mellitus were significantly associated with exudative changes with undefined diagnosis (P<0.001). Of the 4 722 premature neonates (9 444 eyes), 768 eyes were found to have retinopathy of prematurity (8.13%). ConclusionThe detection rates of retinal hemorrhage and exudative changes with undefined diagnosis are relatively high in the neonates in Chongqing. Gender, gestational age, birth weight, mode of delivery and maternal gestational diabetes mellitus are all risk factors of neonatal fundus disease in the neonates.
