摘要:

目的 研究母乳库捐献母乳(donor human milk,DHM)喂养支持系统应用于高危妊娠产妇母乳喂养的效果。方法 回顾性调查2017年1月至2017年12月于我科住院分娩的691例高危妊娠产妇(对照组)以及应用DHM喂养支持系统后的2018年1月至2018年12月696例高危妊娠产妇(研究组)临床资料,对两组产妇于术后3 d 乳胀发生率、术后72 h泌乳延迟发生率、新生儿体质量下降幅度、术后6周纯母乳喂养率等指标进行比较。结果与对照组比较,研究组泌乳延迟发生率降低(P<0.01),产后3 d发生胀乳率也明显降低(13.07% vs 22.87%,P<0.001);研究组产后1 d、出院当天、产后6周纯母乳喂养率高于对照组(68.97% vs 61.94%,P=0.007;67.96% vs 45.88%,P<0.001;57.90% vs 40.96%,P<0.001);研究组新生儿体质量下降幅度明显低于对照组[(158.82±72.99) vs (220.88±77.17)g, t=15.385,P<0.001],新生儿体质量下降人数也低于对照组(P<0.05)。结论DHM喂养支持系统能够促进高危产妇母乳喂养,可降低泌乳延迟及乳胀发生率,降低新生儿体质量下降幅度,适合在临床推广。

Abstract:
Objective To assess the effect of the donor human milk (DHM) supporting system in assisting breast feeding practices for mothers with high-risk pregnancy. MethodsWe retrospectively collected the data from 696 mothers  (study group) with high-risk pregnancy utilizing the DHM supporting system following delivery in our hospital in 2018 and also from 691 mothers  (control group) with high-risk pregnancy who did not use the supporting system following delivery in 2017. We analyzed the occurrence rates of breast distention at 3 d and delayed lactation at 72 h after delivery, weight loss of the infants, and the rates of exclusive breast feeding within 6 weeks after delivery between the 2 groups. ResultsCompared with those in the control group, the mothers in the study group showed significantly lower rates of delayed lactation (P<0.01) and breast distention (13.07% vs 22.87%, P<0.001) at 3 d after delivery, and had significantly higher rates of exclusive breast feeding at 1 d after the delivery (68.97% vs 61.94%, P=0.007), on the day of discharge (67.96% vs 45.88%, P<0.001) and at 6 weeks after delivery (57.90% vs 40.96%, P<0.001). The infants in the study group had significantly lower body weight loss than those in the control group (158.82±72.99 vs 220.88±77.17 g, t=15.385, P<0.001), and the number of infants with body weight loss was also significantly lower in the study group than in the control group (P<0.05). ConclusionThe DHM supporting system can facilitate breast feeding of the infants born to mothers with high-risk pregnancy, and helps to lower the incidences of delayed lactation and breast distension of the mothers and reduce body weight loss of the infants.
