摘要:

目的评估大气压低温等离子体联合生理盐水漂白牙齿的效果及安全性。方法将40颗拔除的人类牙齿完全随机化分为4组,每组10颗。A组:低温等离子体联合生理盐水漂白组;B组:35%过氧化氢凝胶联合冷光源漂白组;C组:35%过氧化氢凝胶漂白组;D组:氩气流联合生理盐水组。同时将40只SD大鼠完全随机化分为4组,处理方法如上。各实验组处理时间均为20 min,使用CIE(国际照明委员会)颜色系统评价各组离体牙漂白效果;使用扫描电镜分析各组离体牙处理后的釉质表面形态。通过牙髓组织病理分析评估不同漂白方法引起的术后牙髓反应。结果低温等离子体联合生理盐水(A组)漂白处理20 min后漂白效果优于35%过氧化氢凝胶漂白组(C组)和氩气流联合生理盐水组(D组,P<0.05),与35%过氧化氢凝胶联合冷光源漂白组(B组)漂白效果相近,差异无统计学意义(P>0.05)。A组牙齿釉质表现为光滑表面上少量的弹坑状凹陷,B组与C组的牙齿釉质处理区域均表现为粗糙表面(蜂窝状凹陷),D组牙齿釉质表现为光滑表面。牙髓组织病理分析显示A组样本多为正常或轻度炎症细胞浸润,B组与C组样本出现了明显的病理改变,如重度炎症细胞浸润或坏死,D组样本表现为正常的牙髓组织。 结论低温等离子体联合生理盐水漂白牙齿效果明显,对釉质表面形态结构及牙髓组织无明显不良影响,为牙齿漂白提供了新的选择。

Abstract:

ObjectiveTo evaluate the bleaching efficacy and safety of nonthermal atmospheric pressure plasma (NAPP) with saline solution (0.9% NaCl). MethodsForty extracted human teeth were randomly divided into 4 groups: Group A (NAPP+saline solution), Group B (cold light+35% hydrogen peroxide gel), Group C (35% hydrogen peroxide gel), and Group D (argon flow+saline solution). Forty Sprague-Dawley (SD) rats were also randomized to Groups 1-4, and were treated with the same grouping method as above. After treatment for 20 min, the bleaching effectiveness was analyzed by the CIELAB color system of International Commission on Illumination (CIE). The surface morphology of the teeth was evaluated by scanning electron microscopy (SEM). The rats were anesthetized, and the upper right first molars were bleached. In 2 d after the experiment, the rats were euthanized, and the maxillae were processed for pathological analysis in order to evaluate the postoperative pulpal responses after bleaching. ResultsCompared with Group C and Group D, Group A showed greater bleaching efficacy (P<0.05), which was similar to that of Group B. The enamel surfaces of the tooth from Group B and Group C became rougher (honeycomb depression), while some small crater-like depressions on a smooth surface were observed in the surface of Group A, but only smooth surface was seen in the enamel surface of Group D. The dental pulp analysis showed severe inflammatory infiltrate or necrosis were seen in Group B and Group C (P<0.05), while normal or mild inflammatory infiltration was found in Group A, and normal pulp issue was observed in Group D. ConclusionNAPP combined with saline solution exhibits good bleaching efficacy and causes fewer pulp alterations and enamel surface changes, and can be used as a new option for tooth bleaching
