摘要:

目的      对比观察剖宫产术后应用地佐辛静脉自控镇痛和硬膜外自控镇痛对血清泌乳素(prolactin,PRL) 浓度的影响和不良反应发生的情况。      方法      2014 年11月至2015年3月,选择拟在解放军401医院行剖宫产术的足月妊娠产妇60 例,采用随机数字表法将产妇均分为3组:地佐辛组、硬膜外组和对照组。地佐辛组术后采用静脉镇痛泵,配方为地佐辛0.5 mg/kg配生理盐水至100 mL;硬膜外组术后采用硬膜外镇痛泵,配方为舒芬太尼20 μg+罗哌卡因125 mg 配生理盐水至100 mL;对照组术后不采用任何镇痛方法。观察各组术后各时点镇痛评分、泌乳始动时间、不良反应情况;采用化学发光法测定术前、术后24 h、术后48 h血清泌乳素浓度。      结果      地佐辛组术后各时点镇痛评分与硬膜外组相比差异无统计学意义(P>0.05),但均低于对照组(P<0.05);地佐辛组与硬膜外组术后24、48 h血清泌乳素浓度相比差异无统计学意义 (P>0.05),但均较对照组显著升高(P<0.05);地佐辛组与硬膜外组泌乳始动时间相比差异无统计学意义(P>0.05),但均明显早于对照组(P<0.05);地佐辛组不良反应发生率低于硬膜外组(P<0.05)。      结论      剖宫产术后采用地佐辛静脉自控镇痛效果良好,有利于提高产妇血清泌乳素浓度,不良反应少。

Abstract:

Objective      To observe the analgesic effect and adverse reactions of patient-controlled intravenous analgesia with dezocine or patient-controlled epidural analgesia on patients undergoing caesarean section and the effect on serum prolactin (PRL).       Methods      A total of 60 pregnant women, who underwent caesarean section in No. 401 hospital of PLA during November 2014 to March 2015, were randomly divided into dezocine group, epidural group and control group. Patients of the dezocine group received 0.5 mg/kg of dezocine in 100 mL saline using a venous self-control analgesic pump after caesarean section. Patients of the epidural group were treated with 20 μg sufentanil and 125 mg ropivacaine in 100 mL saline using a patient-controlled epidural analgesic pump. Patients of the control group did not use any analgesia methods. Visual analogue scale (VAS), initial time of lactation, adverse reactions and occurrences of complications were used to evaluate the effects of different treatments after caesarean section. Serum PRL levels were determined with chemiluminescence assay before operation and at 24 and 48 h after caesarean section.       Results      The VAS scores of the dezocine group and epidural group were significantly lower than those of the control group at postoperative 12, 24 and 48 h (P<0.05), whereas there was no significant difference between the dezocine group and the epidural group (P>0.05). The serum PRL levels of the dezocine group and epidural group at 24 and 48 h after cesarean section were significantly higher than those of the control group (P<0.05), whereas there was no significant difference between the dezocine group and the epidural group (P>0.05). Both analgesic methods could significantly advance the initial time of lactation compared with the control group (P<0.05), whereas the initial time of lactation had no significant difference between the dezocine group and the epidural group (P>0.05). The incidence of adverse reactions in the dezocine group was lower than that in the epidural group (P<0.05).       Conclusion      The patient-controlled intravenous analgesia with dezocine has good curative effect and less adverse reactions for the patients undergoing caesarean section, and can promote the secretion of PRL.
