摘要:

目的      探讨川崎病(kawasaki disease, KD)患儿血清血红素加氧酶-1(heme oxygenase-1, HO-1)与C反应蛋白(C reactive protein, CRP)水平变化对川崎病冠状动脉病变(coronary artery lesions,CAL)的预测作用。      方法      采集KD患儿外周血89例,分为无冠脉病变(NCAL)组35例、扩张内径<5 mm 冠脉病变(CAL<5 mm)组44例,及扩张内径≥5 mm冠脉病变(CAL≥5 mm)组10例,并设置发热对照(F)组34例和健康对照(N)组45例。检测血清丙二醛(malondialdehyde,MDA)、HO-1、超氧化物歧化酶(superoxide dismutase, SOD),并结合CRP和Hb进行分析,对部分患儿IVIG治疗前后以上指标进行对比。      结果      急性期,KD各组MDA水平显著增高;CAL<5 mm组HO-1明显高于其他4组;仅CAL≥5 mm组SOD水平低于N组;CAL≥5 mm组CRP显著高于其他4组。在HO-1≥9 ng/mL的30例KD患者中, 当CRP>44 mg/L时,CAL的发生率为56.52%(13/23),明显高于NCAL(0%,0/7)。在HO-1<9 ng/mL的31例CAL患者中,当CRP>85 mg,CAL≥5 mm的发生率为(88.89%,8/9),明显高于CAL<5 mm的发生率(36.36%,8/22)。CAL组HO-1与Hb呈负相关(r=-0.37, P<0.05)。IVIG治疗后,KD患者H0-1明显升高,仅CAL组MDA水平下降明显,SOD的改变并不显著(P>0.05)。      结论      联合应用HO-1 与CRP的水平变化,可能对于川崎病急性期预测冠脉病变发生与否及严重程度具有重要的意义。

Abstract:

Objective      To determine the roles of serum levels of heme oxygenase-1 (HO-1) and C reactive protein (CRP) in the prediction of coronary artery lesions (CAL) in children with Kawasaki disease (KD).       Methods      Totally 89 children with identified KD admitted in our hospital from March 2013 to May 2014 were enrolled in this study, and were divided into no coronary artery lesion group (NCAL, n=35), CAL<5 mm group (n=44) and CAL≥5 mm group (n=10). Another 34 children with acute febrile diseases (group F, n=34) and 45 normal healthy children (group N, n=45) were also recruited as control. The levels of malondialdehyde (MDA), HO-1 and superoxide dismutase (SOD) were detected before and after intravenous immunoglobulin (IVIG) treatment respectively. Serum levels of CRP and hemoglobin (Hb) were also enrolled to correlation analysis.       Results      During the acute phase of KD, MDA levels were significantly increased. The levels of HO-1 were significantly higher in CAL <5 mm group than the other 4 groups. The levels of SOD were lower in CAL ≥5 mm group than N group. CRP levels in CAL ≥5 mm group were significantly higher than the other groups. The incidence of CAL was 56.52% (13/23) in 30 KD cases with HO-1 ≥9 ng/mL and CRP >44 mg/L, which was significantly higher than that of NCAL (0%, 0/7). The incidence of CAL ≥5 mm was 88.89% (8 of 9) in 31 CAL cases with HO-1 <9 ng/mL and CRP >85 mg/L, which was significantly higher than that of CAL<5 mm (36.36%, 8/22). HO-1 was significantly negatively correlated with Hb level (r=-0.37, P<0.05) in CAL group. IVIG treatment resalted in elevated HO-1 and decreased MDA in CAL group but had no change on SOD activity.       Conclusions      Combined analyzing of HO-1 and CRP may predict the occurrence and severity of CAL in the acute phase of KD.
