摘要:
目的      观察应用不同剂量重组人生长激素(recombinant human growth hormone, rhGH)治疗特发性矮小症(idiopathic short stature,ISS)的疗效。      方法      收集2006-2010年在重庆医科大学附属儿童医院内分泌科及上海交通大学医学院附属瑞金医院儿科就诊的ISS患者106例(男性52例,女性54例)。青春分期:Tanner Ⅰ期75例,Ⅱ期31例。按照生长激素应用的剂量分为3组:低剂量组(每周0.26 mg/kg)、中剂量组(每周0.35 mg/kg)和高剂量组(每周0.41 mg/kg),每组分别为39、23例及44例。每周7 d睡前皮下注射rhGH,疗程1~2年。评价治疗前后身高(height, Ht)、生长速度(growth velocity, GV)、身高标准差积分(height standard deviation score, Ht SDS)、骨龄(bone age, BA)、预测成年身高(predictive adult height, PAH)变化及副作用等指标。      结果      ①疗效观察:rhGH治疗第1年,3组患者GV由治疗前的(4.59±1.45)cm/年、(3.65±1.81)cm/年、(4.5±0.67)cm/年,分别提高到(8.22±1.93)cm/年、(9.97±1.95) cm/年、(11.48± 1.63)cm/年,P<0.01。rhGH治疗第2年,3组患者GV差异无统计学意义(P>0.05)。②骨龄进展:3组治疗1、2年骨龄的进展差异无统计学意义(P>0.05)。③不良反应:高剂量组有6例在治疗3个月后出现暂时性空腹血糖升高,分别为5.7、6.0、6.0、5.9、5.6、6.0 mmol/L,停用GH后血糖恢复正常。个别病例在治疗过程中出现膝部疼痛,不伴随其它临床和实验室异常,考虑生长痛,补充钙剂后疼痛消失。      结论      rhGH治疗对ISS促生长效应效果明显,短期追赶性生长呈剂量依赖性;短期治疗不影响生长潜能;副反应偶见,剂量相对偏高者多见。
Abstract:
Objective      To assess the effect of different doses of recombinant human growth hormone (rhGH) on the growth velocity in Chinese children with idiopathic short stature (ISS).       Methods      A total of 106 ISS patients (52 males and 54 females) were enrolled in the study from 2006 to 2010, with 75 cases of Tanner stage Ⅰ and 31 of stage Ⅱ. They were divided into 3 groups according to the doses of rhGH: a low dose group (0.26 mg/kg every week), a middle dose group (0.35 mg/kg every week) and a high dose group (0.41 mg/kg every week) with 39, 23 and 44 patients, respectively. They were given rhGH by subcutaneous injection every day  for 1 to 2 years. The changes of height (Ht), growth velocity (GV), height standard deviation score (HtSDS), bone age(BA), predicted adult height (PAH) and side effects were recorded.       Results      (1) The GV of 3 groups after 1 year treatment was enhanced to 8.22±1.93, 9.97±1.95 and 11.48±1.63 cm, respectively, versus 4.59±1.45 cm, 3.65±1.81 cm and 4.5±0.67 cm, respectively before treatment (P<0.01). The high dose group gained higher GV than the low and middle dose groups (P<0.01). As for the GV of the 3 groups after 2 years, there was no statistically significant difference (P>0.05). (2) The bone age progress of the 3 groups after 1 and 2 years’ treatment showed no statistically significant difference (P>0.05). (3) After 3 months of treatment, 6 cases of the high dose group had mild high fasting blood glucose with 5.7, 6.0, 6.0, 5.9, 5.6, and 6.0 mmol/L, respectively, but came back to normal level after short period of stopping rhGH. Knee pain occurred in occasional individuals without bone and biochemical abnormalities.       Conclusion      RhGH has a great effect on growth for ISS, with obvious short-term catch-up growth in a dose-dependent manner. Short-term treatment does not affect the growth potential. Occasional side effects are relatively more common in high dose groups. Long term follow-up is needed.
