摘要:
目的      探讨G蛋白偶联受体48(G protein coupled receptor 48,GPR48)和Toll样受体4(toll like receptor 4,TLR4)在大鼠激素性股骨头坏死组织中的表达变化及其相关性,阐明GPR48与TLR4在激素性股骨头坏死进程中的调控作用。      方法      健康SD大鼠68只,按随机数字表法分为实验组(44只,肌注地塞米松10 mg/kg,每周1次)和对照组(24只,注射等体积生理盐水代替激素),于2、6、8、10周时分别获取实验组11只和对照组6只大鼠的双侧股骨头,进行组织学观察(HE、Masson三色染色)、免疫组化及RT-PCR检测。      结果      ①HE染色显示,实验组2周可见骨小梁微小断裂,10周骨小梁断裂范围扩大,形成大量空骨陷窝;Masson三色染色及胶原含量分析显示,与对照组相比,实验组单位面积内胶原含量从第6周开始下降,第10周明显下降,差异有统计学意义(P<0.05),实验组组内各个时间点的比较差异有统计学意义(P<0.05),胶原含量呈逐渐下降趋势;②免疫组织化学染色分析:实验组6、8、10周各个时间点GPR48表达的累积光密度(5.87±0.53)、(4.82±0.52)、(3.94±0.49)均低于对照组(P<0.05),并呈逐渐降低趋势;实验组6、8、10周各个时间点TLR4表达的累积光密度(6.93±0.58)、(8.10±0.46)、(8.70±0.95)均高于对照组(P<0.05),并呈逐渐上升趋势;实验组不同时间点GPR48与TLR4表达呈负相关(r=-0.821,P<0.05);③RT-PCR检测:GPR48 mRNA表达在实验组内各个时间点之间呈逐渐降低趋势,而TLR4 mRNA表达则呈逐渐上升趋势;实验组不同时间点GPR48与TLR4 mRNA表达呈负相关(r=-0.791,P<0.05)。      结论      大鼠激素性股骨头坏死发展进程中GPR48和TLR4的表达呈负相关性,共同介导坏死过程中的炎症免疫反应。
Abstract:
Objective      To analyze G protein-coupled receptor 48 (GPR48) and toll-like receptor 4 (TLR4) expression changes and regulation activities in steroid-induced avascular necrosis of the femoral head in rats.        Methods      Sixty-eight SD rats were randomly divided into 2 groups, an experimental group (44 rats injected with dexamethasone sodium phosphate 10 mg/kg weekly) and a control group (22 rats injected with an equal volume of normal saline). At week 2, 6, 8, and 10, rats were sacrificed (11 rats in the experimental group and 6 rats in the control group at each time point) to obtain femoral heads, followed by histological observation, immunohistochemistry assay and RT-PCR detection.        Results      (1) HE staining results revealed that the experimental group showed occasionally small micro-fractures at week 2 and displayed a wide range of trabecular bone fracture at week 10. Masson trichrome staining results revealed that the per unit area of collagen content in the experimental group was significantly higher at weeks 6, 8 and 10 than that in the control group (P<0.05). (2) Compared with the control group at weeks 6, 8 and 10, the experimental group showed significantly decreased GPR48 level (5.87±0.53, 4.82±0.52, and 3.94±0.49, respectively, P<0.05) and increased TLR4 level (6.93±0.58, 8.10±0.46 and 8.70±0.95, respectively, P<0.05). It displayed a negative correlation between GPR48 and TLR4 expression (r=-0.821, P<0.05) at different time points. (4) Compared with the control group, GPR48 mRNA expression in the experimental group was markedly decreased while TLR4 mRNA was increased in a time-dependent manner (P<0.05). It also displayed a negative correlation between GPR48 and TLR4 mRNA levels (r=-0.791, P<0.05) at different time points.       Conclusion      GPR48 and TLR4 expression is negatively correlated and mediates the inflammation and immune reaction in the progression of steroid-induced avascular necrosis of the femoral head in rats.
