目的      动态测定急性缺血性卒中(acute ischemic stroke,AIS) 患者外周循环中内皮祖细胞(endothelial progenitor cells, EPCs)的数量,并探讨其可能的动员机制。      方法      发病后12 h入院的71例AIS患者,采用流式细胞仪测定外周血EPCs含量,以CD133/KDR作为EPCs表面标记。时间点分别为病后第1、7、14、21天。同时采用酶联免疫吸附法测定血浆基质细胞衍生因子-1(serum stromal cell-derived factor-1,SDF-1)的浓度。对照组包括42例同期招募的健康志愿者。      结果      AIS患者基线EPCs水平低于对照组[(0.022±0.013) vs (0.051±0.020),P<0.01]。血压、甘油三酯、低密度脂蛋白和空腹血糖与EPCs水平呈负性相关(P<0.01)。收缩压和低密度脂蛋白是EPCs基线水平的独立预测因素。AIS后第7天循环EPCs数量增加,第14天达高峰,第21天开始下降。血浆SDF-1浓度呈类似变化。此外,EPCs变化值(EPCs14d-EPCs1d)与脑梗死体积(r=0.708,P<0.01)或第14天的SDF-1浓度相关(r=0.774,P<0.01)。      结论      AIS患者的EPCs基线水平可能反映累积的血管内皮损伤,而AIS后外周血EPCs数量增加来自于EPCs动员,其机制可能与SDF-1表达上调有关。
Abstract:
Objective      To demonstrate the mobilization of endothelial progenitor cells (EPCs) after acute ischemic stroke (AIS) and investigate its possible mechanism.       Methods      A total of 71 patients with AIS were examined at days 1, 7, 14 and 21 after stroke onset. Forty-two healthy controls were also examined. Circulating EPCs were defined by the surface markers CD133/KDR and analyzed by flow cytometry. Serum stromal cell-derived factor-1 (SDF-1) concentrations were determined by enzyme-linked immunosorbent assay.       Results      Baseline EPCs level was significantly lower in patients with AIS than in control subjects (0.022±0.013 vs 0.051±0.020, P<0.01). Blood pressure, triglyceride, low-density lipoprotein (LDL) and fasting blood sugar were negatively correlated with EPCs levels (P<0.01). Systolic blood pressure and LDL remained independent predictors of baseline EPCs levels. The number of circulating EPCs increased on day 7 after AIS, reached peak level on day 14 and started to decrease on day 21. The concentration of SDF-1 had similar changes. The increment of EPCs (absolute difference between the number of EPCs on day 14 and admission) was correlated with the infarct volume (r=0.708; P<0.01) and SDF-1 concentration on day 14 (r=0.774, P<0.01).       Conclusions      Baseline EPCs level in patients with AIS may reflect the cumulative vascular endothelial damage, while increase of circulating EPCs results from the mobilization of EPCs in response to stroke stress. The mechanism may be associated with up-regulation of SDF-1 expression.
