目的      测定肥胖患者皮屑中钠、钾离子浓度及皮肤组织钠、钾转运相关蛋白的表达,并探讨其临床意义。      方法       招募127例重庆地区自愿者,并以体质指数(BMI)和腰围(WC)值,将其分为肥胖、超重、健康组和腹型肥胖、健康组,刮取研究对象双前臂内侧皮屑,用消化液(1.5 mL×65%硝酸和0.5 mL×70%高氯酸)加热溶解后,使用火焰原子分光光度计检测溶液中钠、钾的含量;提取小鼠及人皮肤组织蛋白,用蛋白印迹法检测糖皮质激素受体、醛固酮受体、Na+-K+-ATP酶表达。      结果      在最适反应条件下,本方法的相对标准偏差在0.34~1.45,回收率为93.1%~107.2%;肥胖组与健康组相比,皮屑钠、钾值均有显著性差异(P=0.002,0.000),肥胖组与超重组比差异也有显著性(P=0.002,0.000),超重组与健康组比较均无差异。Western blot检测小鼠和人皮肤组织中糖皮质激素受体、醛固酮受体、Na+-K+-ATP酶3种蛋白均有表达。      结论       应用火焰原子分光光度计测量肥胖者皮屑钠、钾含量明显升高,可简易地反映临床肥胖患者组织电解质变化,钠、钾转运相关的3种蛋白在小鼠和人皮肤组织中均有表达。
Abstract:
Objective      To determine the concentrations of sodium and potassium in the dander from obese patients, and the expression of sodium- and potassium-related transporters at skin tissue, and to explore their clinical significance.       Methods      A total of 127 volunteers, 75 females and 52 male with an average age of 52.83±6.11 (ranging from 40 to 60) were recruited from Chongqing region. The volunteers were divided into normal weight (n=50), overweight (n=56) and obese group (n=21) by their body mass index (BMI), or non-abdominal obesity (n=81) and abdominal obesity group (n=46) by their waist circumference (WC). Dander scraped from medial forearm of subjects was digested by heated solution (1.5 ml 65% nitric acid and 0.5ml 70% perchloric acid), then the concentration of sodium and potassium in the solution was detected by flame atomic spectrophotometry. Glucocorticoid-receptor, aldosterone receptor and Na+-K+-ATP enzyme in skin tissue samples from C57 mice and from human during operation were detected by Western blotting.       Results       At optimal reaction conditions, our method obtained a relative standard deviation ranging from 0.34 to 1.45 and a recovery rate ranging from 93.1% to 107.2%. Obese group had a significantly higher contents of sodium (P=0.002) and potassium (P=0.000)  when compared with normal weight group and overweight group. However, no significant difference was observed between normal weight group and overweight group. Western blotting showed that glucocorticoid-receptor, aldosterone receptor and Na+-K+-ATP enzyme were all expressed at mice and human skin tissue.       Conclusion      We establish the method to determine the dander contents of sodium and potassium from human skin tissue, and find increased concentrations of sodium and potassium in dander form obese subjects, suggesting that our method can easily reflect the tissue electrolyte change of obesity. Sodium and potassium transporter-related proteins are all expressed in mouse and human skin tissue.
