摘要:
目的      通过测定骨形态发生蛋白-7(bone morphogenetic protein-7,BMP-7)、转化生长因子-β1(transforming growth factor-β1,TGF-β1)、碱性成纤维细胞因子(basic fibroblast growth factor,bFGF)水平变化,探讨丹参促进骨折愈合在分子水平的作用机制。      方法        选取72只新西兰大白兔,采用单纯随机抽样方法分为实验组和对照组,每组36只,制作左前臂桡骨骨折模型。实验组肌注丹参注射液(每天1次,每次2 mL),对照组注射生理盐水(每天1次,每次2 mL),至术后28 d。2组分别于术后第1、2、3、4、6、8周选取6只大白兔,X线片观察骨折愈合情况,ELISA法检测其血清BMP-7、TGF-β1和bFGF浓度,处死后取以骨折处为中心的4 mm组织标本,HE染色和免疫组织化学方法检测BMP-7、TGF-β1和bFGF在局部组织中的表达。      结果        ①术后第3、4周X线片显示,与对照组比较,实验组骨痂生成量增加,且达到骨性愈合的时间较早。②ELISA法检测结果显示,术后第4周实验组BMP-7血清含量明显高于对照组(P<0.01);第2周实验组bFGF血清含量明显高于对照组(P<0.05);第3周实验组TGF-β1血清含量明显高于对照组(P<0.05)。③HE染色结果显示,术后第3、4周实验组骨痂形成增多,成骨细胞和破骨细胞数量均较对照组多。④术后第3周实验组BMP-7免疫组化平均光密度值明显大于对照组(P<0.05);术后第2周实验组bFGF免疫组化平均光密度值明显大于对照组(P<0.01);术后第3周实验组TGF-β1免疫组化平均光密度值明显大于对照组(P<0.05)。      结论        丹参注射液能促进骨折愈合过程中BMP-7、TGF-β1及bFGF生长因子的表达。
Abstract:
Objective       To observe the changes of bone morphogenetic protein-7 (BMP-7), trans-forming growth factor-β1(TGF-β1) and basic fibroblast growth factor (bFGF) during the healing of fracture in rabbits treated with Danshen injection (an injection containing Salvia miltiorrhiza extract).       Methods      Seventy-two New Zealand White rabbits were made into fracture models and were randomly divided into an experimental group (Danshen injection, 2 mL/d for 28 d) and a control group (normal saline, 2 mL/d for 28 d). The healing of fracture was evaluated by X-ray and the serum levels of BMP-7, TGF-β1 and bFGF were determined by ELISA at 1, 2, 3, 4, 6 and 8 weeks after the operation. Tissues of fracture region were stained by HE staining, and immunohistochemical method was used to detect the expression of BMP-7, TGF-β1 and bFGF.       Results      X-ray results showed that as compared with the control group, bone union appeared earlier and more bone callus formed in the experimental group at 3 and 4 weeks. HE staining results showed that more bone callus formed and more osteoblasts and osteoclasts appeared at 3 and 4 weeks in the experimental group than in the control group. ELISA results showed the serum levels of BMP-7 (4 weeks), TGF-β1 (2 weeks) and bFGF (3 weeks) were significantly higher in the experimental group than in the control group, while immunohistochemical staining displayed that the expression levels of BMP-7 (3 weeks), TGF-β1 (2 weeks) and bFGF (3 weeks) were significantly higher in the experimental group than in the control group.       Conclusion      Danshen injection can upregulate the expression of BMP-7, TGF-β1 and bFGF during the healing of fracture.
