摘要:
目的      探讨磁敏感加权成像(susceptibility weighted imaging, SWI)技术诊断膝关节半月板撕裂的可行性。      方法       对40名临床怀疑半月板损伤患者,行MRI T1WI和T2WI-FS、SWI序列扫描,以膝关节镜为金标准对照,比较SWI与T1WI及T2WI-FS诊断半月板撕裂的敏感性、特异性、准确性和诊断效果。      结果       T1WI序列敏感性为76.3%,特异性为50.0%,准确性为75.0%;T2WI-FS序列敏感性为71.1%,特异性为100.0%,准确性为72.5%;SWI序列敏感性为94.7%,特异性为50.0%,准确性为92.5%。T1WI序列和T2WI-FS序列与SWI相比一致性较差,Kappa值分别为0.391和0.288,诊断效果有显著性差异(P<0.05)。      结论       SWI序列能够应用于膝关节半月板撕裂的诊断,且敏感性和准确性优于T1WI序列和T2WI-FS序列。
Abstract:
Objective        To study the feasibility of susceptibility weighted imaging in diagnosis of meniscus tear of knee joint.       Methods        Forty patients with suspected meniscus tear of knee joint underwent MRI scanning of T1WI, T2WI-FS and SWI sequences. The sensitivity, specificity, accuracy and diagnostic outcome of T1WI, T2WI-FS and SWI sequences were compared with arthroscopy as the golden standards.       Results        The sensitivity, specificity and accuracy of T1WI, T2WI-FS and SWI sequences were 76.3%, 50.0% and 75.0%, respectively; 71.1%, 100.0% and 72.5%, respectively; and 94.7%, 50.0% and 92.5%, respectively, for the diagnosis of meniscus tear of knee joint. The consistency of T1WI and T2WI-FS sequences was poorer than that of SWI sequence in diagnosis of meniscus tear of knee joint (P<0.05). The Kappa value was 0.391 and 0.288, respectively, for T2WI-FS and SWI sequences. The diagnostic outcomes of meniscus tear of knee joint were significantly different between T2WI-FS and SWI sequences (P<0.05).       Conclusion        The SWI sequence can be used in diagnosis of meniscus tear of knee joint, and its sensitivity and accuracy are better than the T1WI and T2WI-FS sequences.
