摘要:
目的     探讨TNF-α抑制剂在大鼠蛛网膜下腔出血(subarachnoid hemorrhage,SAH)后早期脑损伤中的保护作用及其机制。     方法       健康成年雄性SD大鼠48只,体质量250~300 g,按随机数字表法分为假手术组、SAH组、安慰剂组和TNF-α抑制剂组。采用经视交叉前池注血法建立SAH模型,各组于处理后24 h,对实验动物临床行为进行评估;应用免疫组化方法检测大鼠脑皮层TNF-α表达;应用Western blot方法检测大鼠脑皮层MMP-9蛋白表达。     结果       免疫组化显示:SAH组大鼠皮层神经元TNF-α表达明显升高,显著高于假手术组;TNF-α抑制剂组TNF-α表达明显下降(P<0.01)。Western blot检测结果显示:假手术组大鼠MMP-9蛋白表达较低[(2.03±0.21)%];SAH组大鼠皮层MMP-9含量明显升高[(98.92±8.33)%,P<0.01];TNF-α抑制剂组MMP-9蛋白表达降低[(52.12±6.25)%],与SAH组相比差异有统计学意义(P<0.01)。临床行为评估显示:TNF-α抑制剂组大鼠临床行为得到明显改善(P<0.01)。     结论       大鼠SAH后早期脑损伤过程中TNF-α通过调节MMP-9蛋白表达参与SAH后早期脑损伤的病理过程。TNF-α抑制剂能够改善大鼠临床行为,对早期脑损伤发挥保护作用。
Abstract:
Objective       To investigate the role of TNF-α inhibitor during early brain injury after subarachnoid hemorrhage in rats.      Methods       Forty-eight adult male SD rats were randomly and equally divided into 4 groups, sham-operation group, subarachnoid hemorrhage group, vehicle group and TNF-α inhibitor group. TNF-α inhibitor (monoclonal antibody) or PBS of 10 μl was injected into lateral cerebral ventricle of corresponding rats in 30 min before subarachnoid hemorrhage model establishment. Then the model was induced by injecting autologous arterial blood into the prechiasmatic cistern. After general condition and nerve function were evaluated in 24 h after model establishment, all rats were killed and their brain was resected. Expressions of TNF-α in the cortex were observed by immunohistochemistry, and the protein expression was examined by Western blotting. The clinical assessment was also examined. Brain Evan’s blue contents in rat brains were measured at same time points by formamide extraction method.      Results       Immunohistochemistry showed that the expression of TNF-α was increased dramatically in SAH group, which was higher than that of sham group, but that was decreased significantly in TNF-α inhibitor group. Western blotting showed MMP-9 expression was lower in sham group, and higher in SAH group in the cortex (P<0.01). Compared to SAH group, the expression of MMP-9 was decreased significantly in TNF-α inhibitor group (P<0.01). The clinical assessment showed that the clinical behavior of rats was improved significantly in TNF-α inhibitor group (P<0.01).      Conclusion      TNF-α inhibitor improves the clinical assessment of SAH rats and protects early brain injury, suggesting that TNF-α may be involved in the pathological process of early brain injury after subarachnoid hemorrhage by regulating the expression of MMP-9.
