摘要:
目的    探讨人肝细胞癌组织中PPAR-γ和PTEN蛋白的表达及其临床意义。    方法    应用免疫组织化学方法检测78例肝细胞癌及21例正常肝组织中PPAR-γ和PTEN蛋白的表达,分析其与肝细胞癌临床病理特征及预后的关系。    结果    肝细胞癌组织中PPAR-γ蛋白的阳性表达率为51.3%,显著高于正常肝组织(19.0%,P<0.05),PTEN蛋白的高表达率为42.3%,显著低于正常肝组织(90.5%,P<0.05)。PPAR-γ蛋白的表达与肿瘤直径、门静脉癌栓及TNM分期有关(P<0.05);PTEN蛋白的表达与肿瘤直径、门静脉癌栓、淋巴结转移及TNM分期有关(P<0.05,P<0.01)。PPAR-γ与PTEN蛋白表达无明显相关性(r=-0.100, P=0.384)。PTEN蛋白低表达患者术后生存率低于高表达患者(P=0.000);PPAR-γ蛋白的表达与患者术后生存率无关(P=0.175)。COX模型多因素分析结果显示,TNM分期是影响肝细胞癌预后的独立因素。    结论    肝细胞癌组织中PPAR-γ和PTEN蛋白的表达水平与肝细胞癌的恶性生物学行为密切相关,PTEN有助于评价患者预后。
Abstract:
Objective    To explore the expression of peroxisome proliferator-activated receptor γ (PPAR-γ) and phosphatase and tensin homology deleted on chromosome ten (PTEN) protein in hepatocellular carcinoma (HCC), and to elucidate their clinical significance.     Methods    Immunohistochemical analysis was performed to detect the expression of PPAR-γ and PTEN protein in 78 specimens of HCC tissues and 21 specimens of normal hepatic tissues. The relationship of their expression with the clinicopathological characteristics and prognosis of HCC was analyzed.     Results    The positive expression rate of PPAR-γ (51.3%) in HCC tissues was significantly higher than that (19.0%) in normal hepatic tissues (P<0.05). The high expression rate of PTEN in HCC tissues (42.3%) was significantly lower than that (90.5%) in normal hepatic tissue (P<0.05). The PPAR-γ expression was correlated with tumor size and portal vein tumorous thrombus (P<0.05) as well as tumor-node-metastasis (TNM) stage (P<0.01). The PTEN expression was correlated with tumor size, portal vein tumorous thrombus, and invasion or lymph node metastasis (P<0.05) as well as TNM stage (P<0.01). No significant correlation was observed between PPAR-γ expression and PTEN expression in HCC tissues (r=-0.100, P=0.384). Patients with low PTEN expression had significantly worse postoperative survival than those with high PTEN expression (P=0.000, Log-rank test). However, PPAR-γ expression was not significantly correlated with the postoperative survival of HCC patients (P=0.175, Log-rank test). A multivariate analysis based on COX regression model demonstrated that TNM stage was an independent factor affecting prognosis of HCC.     Conclusion    The expression levels of PPAR-γ and PTEN protein are closely related to the malignant biological behavior of HCC. PTEN is helpful to evaluate prognosis.
