目的   了解血吸虫病潜在流行区——重庆三峡库区血吸虫病防治能力现状及影响因素,发现工作中的问题和困难。   方法    采用多阶段分层整群抽样,抽取万州、开县及云阳3个区县卫生人员进行问卷调查和定性访谈。问卷调查主要研究一般情况如年龄、学历、血防工作经历、血防治知识知晓、培训等。定性访谈主要探讨血防工作中的困难、问题及建议。对卫生人员基本情况进行统计描述,对不同性别、年龄、职业等的卫生人员血防知识全部正确率进行χ2检验。访谈录音用定性分析软件MAXqda,运用主题框架分析法进行分析。   结果    问卷调查180名,访谈25名卫生人员。93.33%为大专或以下学历,73.33%未从事过血吸虫病防治工作,血吸虫病防治工作没有专职人员。47.78%对基本血防知识缺乏了解,影响因素为职业和学历。75.56%在最近1年未参加过培训。流动人口及本地居民的血清学监测难以开展,钉螺监测人员不足,缺乏专业技能。对居民的健康宣传限于在开展传染源或螺情监测时发放传单,学生的血吸虫病健康教育尚未开展。政府投入有限,部门间合作机制尚未形成。   结论    三峡库区的医疗卫生人员血防知识知晓率低,缺乏培训。传染源及钉螺监测,居民健康教育已逐步开展。需加强政府领导及投入,强化血防队伍防治能力建设,可派卫生人员到疫区学习。亟须有效完成各项监测,多形式开展健康教育,多部门合作,形成群防群控的良好局面。
Abstract:
Objective    To investigate the problems and difficulties in prevention and control of schistosomiasis by investigating the status quo of schistosomiasis prevention and control ability of health professionals in Three Gorges Dam Reservoir area of Chongqing (China) and its influence factors.    Methods    General data about health professionals in Wanzhou district, Kaixian County and Yunyang County were collected with questionnaires, qualitative interviews, and multi-stage stratified cluster sampling method, including their age, experience and knowledge in schistosomiasis prevention and control, training, difficulties and problems encountered in their work, as well as their suggestions, which were analyzed by chi-square test and theme framework analysis, respectively, using the MAXqda software.    Results    Of the 180 health professionals invested with questionnaires and 25 with interviews, 93.33% graduated from college or had a lower education, 73.33% had not any experience in schistosomiasis prevention and control, 47.78% were quite poor in basic knowledge about schistosomiasis prevention and control, and 75.56% did not receive any training in the past year. It was difficult to conduct serological examination in migrants and local inhabitants. The number of health professionals was not enough to monitoring oncomelania. Health education was limited to spread pamphlets about the sources of infection. The financial support from government was limited with no cooperation between government departments.    Conclusion   The knowledge about schistosomiasis prevention and control is poor in health professionals of Three Gorges Reservoir area of Chongqing. Health professionals lack of training. Monitoring of sources infection and oncomelania as well as health education have begun. More financial support from government is demanded. The ability of health professionals should be strengthened. Health professionals shall be sent to the epidemic areas to get experience. Health education should be conducted and comprehensive measures should be taken against schistosomiasis spread.
