摘要:
目的   观察临床负压创缘治疗(negative pressure wound therapy, NPWT)对创缘及创缘的影响,探讨负压创缘治疗促进慢性创缘血管生长的可能机制。   方法   临床选取慢性创缘病例12例,完全随机分为实验组和对照组,实验组为负压创缘治疗组,对照组为传统方法治疗组,即常规换药(生理盐水敷料)组。分别于治疗前1 d,治疗后1、4、7 d取创缘全层皮肤组织,逆转录聚合酶链反应(reverse transcription polymerase chain reaction, RT-PCR)测样本组织低氧诱导因子-1α  mRNA(hypoxia inducible factor-1α mRNA,HIF-1α mRNA)的表达,免疫组织化学(immunohistochemistry, IHC)测样本组织的HIF-1α及血管内皮生长因子(vascular endothelial growth factor , VEGF)的表达。   结果   实验组HIF-1α mRNA相对含量,HIF-1α、VEGF的阳性表达细胞数均高于对照组,差异有统计学意义(P<0.05),VEGF阳性表达与HIF-1α阳性表达呈正相关(r=0.854,P<0.01,n=48)。   结论   负压创缘治疗能诱导HIF-1α高表达,通过HIF-1α上调VEGF的表达促进创缘血管生长,进而促进创缘愈合。
Abstract:
Objective   To investigate the impact of negative pressure wound treatment (NPWT) on the wound and its circum and explore the possible mechanism by which NPWT promotes the growth of chronic wound blood vessels.    Methods   Totally 12 patients with chronic wounds were subjected in our hospital, and then randomly divided into experimental and control group. The experimental group were treated with NPWT whereas the control group were treated with the traditional method, namely conventional dressing with normal saline dressing. Full-thickness skin samples on the verge of the wound were collected respectively in 1 d before the treatment and 1, 2 and 7 d after the treatment respectively. The mRNA expression of the hypoxia inducible factor-1α (HIF-1α) were detected by reverse transcription polymerase chain reaction (RT-PCR). The protein expressions of HIF-1α and VEGF were detected by immunohistochemical method (IHC).    Results   The mRNA expression of HIF-1α was significantly more in the experimental group than in the control group (P<0.05), so did the protein expression (P<0.05). There was more VEGF in the experimental group than in the control group, with significant difference (P<0.05). Meanwhile, the expression of VEGF and the expression of HIF-1α was positively correlated (r=0.854,P<0.01).    Conclusion   NPWT upregulates the expression of HIF-1α, then increases the expression of VEGF to promote wound blood vessel growth and consequently wound healing.
