目的   探讨乳腺癌循环肿瘤细胞(circulating tumor cell, CTC)与原发肿瘤临床病理特征的关系。   方法   128例乳腺癌及30例乳腺纤维腺瘤患者首次住院时抽取静脉血5 ml,用密度梯度离心法富集分离单个核细胞,以免疫荧光法检测所获单个核细胞中细胞角蛋白(cytokeratin, CK)8/18阳性的CTC,分析CTC与患者临床病理特征的关系。   结果   在128例乳腺癌患者中CTC阳性率为40.6%(52/128),30例乳腺纤维腺瘤患者均未检出CTC。不同淋巴结病理分期(pN0、pN1、pN2和pN3期)患者CTC阳性率分别为15.4%(6/39)、27.0%(10/37)、64.7%(11/17)和66.7%(12/18),各组CTC阳性率差异有统计学意义(P=0.000 6);转移性(M1期)乳腺癌患者CTC阳性率为80%(16/20),较非转移性(M0期)乳腺癌患者CTC总阳性率33.3%(36/108)显著升高 (P=0.000 9);不同TNM分期(Ⅰ、Ⅱ、Ⅲ和Ⅳ期)患者CTC阳性率分别为12.5%(3/24)、32.0%(16/50)、50.0%(17/34)和80.0%(16/20),各组CTC阳性率差异有统计学意义(P=0.000 3)。   结论   部分乳腺癌患者血液中存在CTC。CTC与淋巴结转移、远处转移及TNM分期密切相关。检测CTC有利于进行更精确的病情评估。
Abstract:
Objective   To investigate the relationship between the presence of circulating tumor cells (CTCs) and the clinicopathologic characteristics of primary tumor of breast cancer patients.    Methods   Samples of peripheral blood (5 ml) from 128 cases of patients with breast cancer and 30 cases of patients with breast fibroadenoma hospitalized for the first time were enriched by density gradient centrifugation for mononuclear cells, and then the cells were detected by immunofluorescence with anti-cytokeratin8/18 antibody. The relations between the positive rate of CTC and clinicopathologic characteristics of breast cancer patients were analyzed.    Results   The positive rate of CTC in breast cancer patients was 40.6% (52/128), while CTC was not detected in any patient with breast fibroadenoma. The positive rates of CTC in patients at pathologic lymph node stages (pN0, pN1, pN2 and pN3) were 15.4% (6/39), 27.0% (10/37), 64.7% (11/17) and 66.7% (12/18), respectively. There was significant difference in positive rate of CTC between patients at different pN stages (P=0.000 6). The positive rate of CTC in metastatic breast cancer patients (80.0%, 16/20) was significantly higher than that in non-metastatic breast cancer patients (33.3%, 36/108) (P=0.000 9). The positive rate of CTC in patients at TNM stage Ⅰ, Ⅱ, Ⅲ and Ⅳ was 12.5% (3/24), 32.0% (16/50), 50.0% (17/34) and 80.0% (16/20), respectively. There was significant difference in positive rate of CTC between patients at different TNM stages (P=0.000 3).    Conclusion   The presence of CTCs in peripheral blood is correlated positively with lymph mode metastasis, distant metastasis and TNM stage. CTC detection is useful for more accurate risk assessment of patients with breast cancer.
