摘要:
目的    应用64层螺旋CT研究兔坠落式颅脑减速伤的伤情特点。    方法    实验用兔36只,包括对照组(6只)和致伤组(30只)。采用自行设计的减速伤致伤装置复制兔坠落式颅脑减速性损伤,致伤组根据不同致伤高度分为Ⅰ组(2.5 m)、Ⅱ组(3.5 m)和Ⅲ组(7 m)。于致伤后3~4 h行64层螺旋CT平扫,观察兔颅脑减速伤的CT表现,并与大体和显微解剖学观察结果、高速摄像系统所见进行对照。    结果    坠落高度越高,兔颅脑损伤越严重;蛛网膜下腔出血是最常见的颅内病灶;脑挫裂伤位于撞击处及对冲部位,以对冲部位更严重;颅底骨折均位于对冲部位。CT平扫可准确显示颅骨骨折,且与高速摄像系统所记录的兔颅脑撞击部位一致。对兔颅脑损伤严重的Ⅲ组,CT平扫基本上显示了所有颅脑损伤情况;Ⅱ组轻度损伤者CT扫描呈假阴性;Ⅰ组颅脑CT表现均为阴性。    结论    64层螺旋CT平扫可显示重度及部分中度兔坠落式颅脑减速伤的伤情特点,能为颅脑损伤机制的基础研究提供有价值的信息。
Abstract:
Objective    To study the nueroimaging features of deceleration brain injury after falls in rabbits by 64-slice multidetector CT.     Methods    The deceleration brain injury was produced in rabbits by a self-made impactor setup. Totally 30 rabbits were divided randomly and equally into 3 groups according to the free-falling heights (2.5, 3.5, and 7 m respectively), and another 6 rabbits served as control group. Noncontrast CT scanning (64-slice multi-detector) was performed in 3 to 4 h after trauma, and CT image features of deceleration brain injury of rabbits were compared with macroanatomy, microanatomy and high speed camera records.     Results     The severity of brain injury of rabbits was increased with fall height. Subarachnoid hemorrhage was the commonest intracranial lesion. Contusion and laceration of brain located at the site of impact and contra-coup (away from the site of impact), and the latter was more severe. Basal fracture located at the site of contra-coup. Noncontrast CT displayed the shape and location of skull fracture exactly, and was corresponding with the site of impact recording by high speed camera. On CT scan, almost all of brain injuries were revealed in the highest group, some minor brain injury of rabbits in the moderate group were false negative; and all head of rabbits were negative in the lowest group.     Conclusion    Noncontrast 64-slice multi-detector CT displays the characteristic of severe and partial moderate deceleration brain injury from falls in rabbits. Data obtained by this procedure may provide valuable information for brain injury mechanism.
