摘要:
目的  探讨老年2型糖尿病患者心率变异性(heart rate variability,HRV)与高敏C反应蛋白(high sensitivity C-reactive protein,hs-CRP)的关系。  方法  选取2004-2006年在本科门诊部和住院部治疗的老年2型糖尿病患者30例,再选取老年非糖尿病患者30例,分为糖尿病组和对照组,并行24 h动态心电图检查,测定HRV,包括所有窦性心搏R-R(NN)间期的标准差(SDNN)、全程记录中每5 min NN间期平均值的标准差(SDANN)、相邻NN间期差值的均方根(rMSSD),检测hs-CRP指标,并对糖尿病组的HRV与hs-CRP行线性相关分析。  结果  糖尿病组HRV参数显著低于对照组(P<0.05),而hs-CRP值较对照组显著升高(P<0.05),并且HRV与hs-CRP具有显著的负相关性(r=-0.22, r=-0.21,r=-0.19,P<0.05)。  结论 糖尿病患者心脏自主神经功能的改变和hs-CRP的变化具有相关性。
Abstract:
Objective    To explore the relationship between heart rate variability (HRV) and serum high sensitivity C-reactive protein (hs-CRP) in type 2 diabetic aged patients.     Methods    Totally 30 aged patients with type 2 DM were enrolled in this study who were selected from the outpatients and inpatients of Department of Gerontology in the Third People’s Hospital of Chongqing, and another 30 non-diabetic matched individuals were recruited as control. Their serum hs-CRP and 24 hour-electrocardiography were examined. HRV was assessed by calculating the indices based on statistical operation on RR intervals (time domain analysis) from 24-hour Holter recording, including: standard deviation of normal to normal RR intervals (SDNN), standard deviation of 5-minute mean RR intervals (SDANN) and square root of differences between adjacent NN intervals (rMSSD). Linear correlation analysis was carried out between their serum hs-CRP and HRV.      Results    HRV parameters of type 2 DM patients were significantly lower than those of non-diabetic patients, but hs-CRP values were significantly higher than those of control. Linear correlation analysis indicated that HRV was in a positive correlation with hs-CRP in type 2 diabetic aged patients.     Conclusion    Altered autonomic nervous function may be associated with altered serum hs-CRP in aged patients with type 2 diabetes mellitus.
