摘要:
目的  采用时域、频域及非线性分析方法探讨急进西藏高原初期心率变异性(heart rate variability, HRV)逐日动态变化及其意义。  方法  86例汉族男性新兵在海拔560 m平原完成5 min短程HRV检测后,分为A组(n=12)、B组(n=48)和C组(n=26),飞抵3 675m西藏高原。进藏后,A组于第2天、B组于第3天、C组于第4天分别接受了高原HRV检测。  结果  与平原相比,进藏第2天SDNN、HF、TP均显著下降(P<0.01),rMSSD、PNN50及HFnu也显著下降(P<0.05),而LFnu和LF/HF均显著升高(P<0.01);进藏后第3~4天,SDNN、rMSSD、HF、TP及HFnu呈逐日回升趋势,LFnu及LF/HF呈逐日回落趋势;FD在进藏第2~4天保持低水平波动。  结论  急进高原初期,HRV下降、自主神经系统(ANS)调节功能受到抑制,随后ANS功能呈逐日缓慢恢复至类似平原状态。
Abstract:
Objective  To explore the dynamic changes and significance of heart rate variability (HRV) during the initial phase after acute exposure to Tibet plateau by combining time, frequency domain and non-linear analysis.   Methods  Eighty-six healthy young men of Han nationality underwent short-term (5 min) heart rate variability (HRV) monitoring at 560 meter above sea level (plain), then were transported to Tibet plateau (3675 meter above sea level) by airplane. There, all subjects were divided into 3 groups, and A gorup (n=12) underwent HRV monitoring on the 2nd day after arrival, B group (n=48) on the 3rd day,  C group (n=26) on the 4th day.   Results  Compared with the data obtained at plain, the standard deviation of normal to normal intervals (SDNN), high frequency (HF) and total power (TP) decreased significantly (P<0.01), and root mean square of delta RR (rMSSD), percentage of delta RR>50 ms (PNN50) and normalized high frequency (HFnu) also decreased obviously (P<0.05), but the normalized low frequency (LFnu) and LF/HF increased significantly (P<0.01) in the 2nd day HRV detection at Tibet. During the 3-4 days after arrival at Tibet, SDNN, rMSSD, HF, TP and HFnu gradually increased, while LFnu and LF/HF tended to decrease toward plain values. However, fractal dimension (FD) within the 2-4 days after arrival kept lower than that at plain.   Conclusion  During initial phase of acute exposure to plateau, HRV decreases, and the modulation function of autonomic nervous system is generally blunted, then the function of autonomic nervous system recovers gradually toward plain values.
