摘要:
目的    研究谷氨酰胺预处理在大鼠肝脏缺血再灌注损伤中对肠道黏膜的保护作用并探讨其可能机制。    方法    雄性Wistar大鼠18只分为谷氨酰胺预处理组(Gln组)和生理盐水对照组(对照组),Pringle法阻断肝门致肝脏缺血40 min,去夹再灌注1 h后检测肝功生化酶(ALT、AST、LDH)水平;采取回盲部肠系膜淋巴结(MLN)行细菌培养,检测细菌易位情况;在光、电镜下观察回肠黏膜的组织病理学变化。    结果    Gln组 ALT水平[(407.88±84.06)U/L]显著高于对照组[(312.45±65.83)U/L](P<0.05),AST、LDH两组无显著差异(P>0.05);Gln组MLN细菌含量(492.27±247.52)cfu/g显著低于对照组(3 865.02±1 681.07) cfu/g(P<0.05),选取部分菌落和肠内容物细菌做菌种鉴定,二者具有一致性;镜下显示Gln组小肠黏膜损伤较对照组轻。    结论    谷氨酰胺对肝脏缺血再灌注损伤中的肠道黏膜具有确实的保护作用。
Abstract:
Objective    To investigate the protective effect of glutamine on intestinal mucosa during hepatic ischemia/reperfusion injury in rats and its possible mechanism.     Methods    Eighteen male Wistar rats were randomly receiving glutamine precondition (G group) and 0.9% NaCl solution (control group) for 3 days. The liver was subjected to warm ischemia by cross-clamping the hepatoduodenal ligament for 40 min according to Pringle’s maneuver, then declamping to restore the blood flow. Blood (3 ml) was drawn from abdominal aorta to test ALT, AST, LDH levels; In 1 h after reperfusion, the lymphoid nodes of mesentery in ileocecal part was obtained for germiculture and partial ileac tissues were got to estimate mucosal injury under light microscope and scanning electron microscope.     Results    The ALT level of G group and control group was (407.29±84.27) U/L and (312.67±66.48) U/L respectively, with significant difference (P<0.05), but AST and LDH levels were of no significant difference between two groups (P>0.05).The bacterial content in lymphoid nodes of mesentery from G group was significantly lower than that from control group [(492.27±247.52) cfu/g, (3 865.02±1 681.07) cfu/g, P<0.05].Partial bacterioflora and bacteria inside guts were identified as homologous. After 40-minute ischemia and 1-hour reperfusion,the gut injury in G group was milder than in control group.     Conclusion    Precondition with glutamine intravenously before operation has protective effect on intestinal mucosa during hepatic ischemia/reperfusion injury in rats.
