摘要:
目的    监测脑出血患者颅内压水平,研究降颅压效果与甘露醇用量以及其他影响因素的关系。从而推断甘露醇使用的个体剂量。     方法    无创监测72例脑出血患者每日颅压,采用多元逐步回归方法分析降颅压效果与性别、年龄、出血部位、出血量以及甘露醇的有效用量关系。再将各影响因素进行多元协方差分析。    结果    降颅压效果与性别、年龄无统计学意义(P>0.05),与出血部位、出血量以及甘露醇用量有统计学意义(P<0.05)。幕上出血比幕下出血的降颅压效果好。    结论    可以将脑出血患者入院时的最高颅压,出血部位和出血量,代入公式计算患者所需的甘露醇总用量。
Abstract:
Objective    To study the relationship of decreasing intracranial pressure with mannitol dose and other factors so as to deduce the individual dosage.     Methods    The intracranial pressure of 72 patients was monitored daily by NIP-200 noninvasive intracranial pressure apparatus. The effect of decreasing intracranial pressure, the corresponding mannitol dose, gender, age, bleeding part and volume of hematoma were recorded. These data were applied for multivariate regression analysis, then the influencing factors were applied for analysis of multiple covariance.     Results    The reduction of intracranial pressure was found no statistic significance with gender, age (P>0.05), but statistically related to bleeding part, volume of hematoma and mannitol dose (P<0.05). The reduction of intracranial pressure of supratentorial intracerebral hemorrhage by mannitol was better than that of subtentorial intracerebral hemorrhage.      Conclusion     We can calculate the individual mannitol doses according to patients’ bleeding part, volume of hematoma and the highest intracranial pressure when hospitalized.
