摘要:
目的  探讨乳腺癌的浸润特征及新辅助化疗对乳腺癌浸润特点的改变。  方法  采用全乳大切片技术对比研究病理分期为Ⅰ~Ⅲ期93例行术前化疗与31例未行术前化疗乳腺癌的浸润特征。  结果  对照组皮肤皮下浸润阳性35.5%,发生大导管浸润者25.8%,对肿瘤周围血管浸润者38.7%,肿瘤微血管计数(MVD)17.2±9.5;新辅助化疗后皮肤皮下浸润阳性9.7%,大导管浸润阳性8.6%,肿瘤周围血管浸润阳性11.8%,MVD 8.0±4.4,各项指标均明显低于对照组(P<0.05)。  结论  新辅助化疗显著减轻乳腺癌对周围组织的浸润,可增加保乳机会。
Abstract:
Objective    To study the infiltrating characteristics of breast cancer and the influence of neoadjuvant chemotherapy to it.    Methods    The whole breast subserial section technique was used to study infitrating characteristics in 93 cases of breast cancer with neoadjuvant chemotherapy and 31 cases without from June 2005 to August 2006.     Results    In those without neoadjuvant chemotherapy, skin-subcutaneous tissue involvement was 35.5%, big duct involvement 25.8%, invasion to blood vessel around tumor 38.7% and tumor microvascular density (MVD) was (17.2±9.5). In those with neoadujvant chemotherapy, skin-subcutaneous tissue involvement was 9.7%, big duct involvement 8.6%, blood vessel invasion 11.8% and tumor MVD was (8±4.4). Every index after neoadjuvant chemotherapy was significantly lower (P<0.05).     Conclusion    Neoadjuvant chemotherapy can abate infiltration to peripheral tissues of breast cancer and raise the chance of breast conserving therapy.
