摘要:
目的  探讨犬体外循环(cardiopulmonary bypass, CPB)心肌胰岛素抵抗(insulin resistance, IR)过程中的血清肿瘤坏死因子-α(TNF-α) 水平变化及其作用。 方法  12条健康杂种犬,随机分成Ⅰ组(主动脉阻断30 min,n=6),Ⅱ组(主动脉阻断120 min,n=6)。分别于转流前、开放主动脉后15、45、75 min时采集动脉血标本, 比较相应时点血浆葡萄糖、胰岛素、TNF-α及计算胰岛素抵抗指数(IRI)的变化。 结果  缺血再灌注后血浆葡萄糖、胰岛素及胰岛素抵抗指数不同程度明显增高,于再灌注15~45 min达峰值,心肌对葡萄糖的摄取、利用严重障碍,与Ⅰ组比较,Ⅱ组上述指标增高程度更重、持续更久。同时血清TNF-α明显增加,在主动脉开放15 min达到峰值,并且在75 min仍持续较高水平,较CPB前明显增加,差异有显著性(P<0.05 )。与Ⅰ组比较,Ⅱ组 TNF-α上升更明显、持续更久。 结论  在体外循环中存在明显的心肌IR现象, TNF-α可能是产生心肌IR的重要因素之一。
Abstract:
Objective    To investigate the changes of tumor necrosis factor-α(TNF-α) and  its role  in myocardial insulin resistance (IR) during cardiopulmonary bypass(CPB) in dogs.     Methods    Twelve mongrel dogs undergoing CPB were equally divided into 2 groups at random: groupⅠ,  recelving aortic cross-clamping  for 30 min and group Ⅱ, aortic cross-clamping for 120 min. The plasma concentrations of glucose, insulin, TNF-α and the change of insulin resistance index(IRI) were detected before CPB, reperfusion for 15 min ,45 min and 75 min, respectively.     Results    After myocardial ischemia-reperfusion, the plasma concentrations of glucose, insulin and  IRI  were all significantly increased and reached their peak levels during reperfusion for 15 min and 45 min in comparison with pre-bypass levels (P<0.05) . Meanwhile there was severe dysfunction in myocardial glucose uptaking and utilization. In comparison with group I ,the increased degree of those indexes was more obvious and lasted longer time in groupⅡ. After ischemia reperfusion, TNF-α  increased obviously, reached their peak points 15 min after being declamped, and remained a high level within 75 min as compared with pre-bypass (P<0.05).Compared with groupⅠ, the rise of TNF-α in groupⅡ was more evident and lasted longer.     Conclusion    The phenomenon of myocardial insulin resistance exists after myocardial ischemia-reperfusion during extracorporeal circulation. TNF-α plays an important role in myocardial insulin resistance undergoing cardiopulmonary bypass.
