摘要:
目的  探讨精氨酸营养干预对重型颅脑损伤大鼠免疫功能和蛋白代谢的影响。 方法  建立重型颅脑损伤大鼠模型。40只SD大鼠,雌雄各半,分为3组:正常对照组(C组);普通肠内营养组(EN组);普通肠内营养添加Arg组(Arg组)。各组用等氮、等热卡的营养支持,3、7 d后取外周血标本。应用吞噬白色念珠菌计数法、免疫组化法、放射免疫法和ELISA法,分别测定白细胞吞噬功能、T细胞表型CD4+ 、CD8 +含量、血清白介素-2(IL-2)水平、胰岛素(INS)、胰高血糖素(GIU)、皮质醇(Cor)、视黄醇结合蛋白(BPR)和前白蛋白(PA)的含量。 结果  重型颅脑损伤后两实验组大鼠细胞免疫功能均显著下降,但Arg组的CD4+/CD8+ 、IL-2水平和白细胞吞噬率在各时相点都非常显著高于EN 组(P<0.01)。两实验组的PA、RBP在伤后第3天无显著性差异(P>0.05),第7天,Arg组的PA较EN 组显著升高(P<0.05),RBP和EN 组相比有增高的趋势,但无统计学意义(P>0.05)。两实验组的INS/GLU在伤后第3天无统计学意义(P>0.05),第7天 Arg组则明显高于EN 组(P<0.05)。EN 组的Cor含量在各时相点都明显高于Arg组(P<0.01)。 结论  经肠道补充Arg能有效改善重型颅脑损伤后大鼠免疫功能和蛋白代谢紊乱。
Abstract:
Objective    To study the effect of arginine-enriched enteral nutrition on cellular immunological function and protein metabolism in rats with severe head injury.     Methods    The model of severe head injury was established by local impact injury. Forty SD rats were randomly divided into 3 groups: normal control group (group C), enteral nutrition group (group EN), and EN adding arginine group (group Arg).Rats in group C were not subjected to injury. In groups EN and Arg, they were fed nutritional diets containing the same nitrogen and calorie 4 h after injury. The peripheral blood samples of all rats were collected on the 3rd day and the 7th day. PMN phagocytosis rate was observed by phagocytosis Candida albicans count. CD4+ cells and CD8+ cells were analyzed by SABC. IL-2 level, insulin(INS), glucagon(GLU), and cortisol(Cor) concentrations were measured by RIA. Contents of PA and RBP were examined by ELISA.     Results    CD4+, CD4+/CD8+, IL -2 levels, phagocytosis ratios were noticeably higher in group Arg than group EN (P< 0.01)on each time point. There was no significant difference in PA and RBP contents between group EN and group Arg(P>0.05)on the 3rd day. The PA content of group Arg was higher than that of group EN(P< 0.05), and RBP content of group Arg was greater than that of group EN, but had no statistical significance(P>0.05)on the 7th day after injury. On the 3rd day after injury, there was no significant difference in INS/GLU between group EN and group Arg(P>0.05), but significant difference existed on the 7th day after injury. The Cor content of group Arg was noticeably lower than that of group EN on each time point(P<0.01).     Conclusion    Arginine-enriched enteral nutrition can effectively retard immunosuppression and protein metabolism disturbance in rats with severe head injury.
