摘要:

目的探讨持续性姿势-感知性头晕（persistent postural-perceptual dizziness, PPPD）患者前庭自旋试验（vestibular autorotation test,VAT）的特征。方法 回顾性收集2019年12月至2020年12月在我院神经内科确诊为PPPD的患者47例以及同期确诊为外周性眩晕患者51例、中枢性眩晕患者43例，比较3组患者一般资料构成和VAT检测指标。结果 PPPD组患者平均年龄最小（P<0.01），病程最长（P<0.01）；中枢性眩晕组水平增益增高例数显著高于其他两组（P<0.01）；外周性眩晕组水平增益降低例数显著高于其他两组（P<0.01）；PPPD组非对称性异常例数显著低于其他两组（P<0.05）；外周性眩晕组与中枢性眩晕组水平增益异常合并水平相移异常例数显著高于PPPD组（P<0.01）；PPPD组单纯水平相移异常例数显著高于其他两组（P<0.01）；PPPD组病程与水平相移异常呈正相关关系（r=0.439）。结论PPPD患者VAT检查以单纯水平相移异常为主要特征，而外周性或中枢性眩晕以水平增益异常合并水平相移异常为主，VAT检测可为PPPD的鉴别诊断提供参考。

Abstract:

ObjectiveTo investigate the characteristics of vestibular autorotation test (VAT) in patients with persistent postural-perceptual dizziness (PPPD). MethodsA total of 47 patients with PPPD, 51 patients with peripheral vertigo and 43 patients with central vertigo in our hospital from December 2019 to December 2020 were retrospectively recruited in this study. Their general data and indicators of VAT were compared among the 3 groups. ResultsThe patients from the PPPD group had the youngest average age and longest course of disease when compared with the other 2 groups (both P<0.01). The number of cases with increased horizontal gain was significantly higher in the central vertigo group than the other 2 groups (P<0.01), and the number of those with decreased horizontal gain was obviously higher in the peripheral vertigo group than the other 2 groups (P<0.01). The PPPD group had notably less patients with abnormal asymmetry than the other 2 groups (P<0.05). The number of abnormal horizontal gain combined with abnormal horizontal phase shift was remarkably larger in the peripheral vertigo group and central vertigo group than the PPPD group (P<0.01). There were more patients having simple abnormal horizontal phase shift in the PPPD group than the other 2 groups (P<0.01). Positive correlation was observed between the course of disease and abnormal horizontal phase shift in the PPPD group (r=0.439). ConclusionThe VAT of PPPD patients is mainly characterized by simple horizontal phase shift abnormalities, peripheral or central vertigo is mainly caused by abnormal horizontal gain combined with abnormal horizontal phase shift, VAT can provide reference for differential diagnosis of PPPD.
