摘要:

目的 分析使用胸主动脉腔内隔绝术治疗的胸主动脉穿透性溃疡患者的临床资料,探讨胸主动脉穿透性溃疡患者治疗的适应证及该手术的安全性和有效性。方法 回顾分析2018年10月至2019年5月在本科室完成的胸主动脉穿透性溃疡行胸主动脉腔内隔绝术治疗患者的临床资料。分别对患者年龄、病变部位及类型、伴随疾病、手术方法、手术时间、出血量、住院时间、术中及术后并发症以及术后随访情况进行分析。结果 研究共纳入7例患者,年龄(70.29±7.54)岁,其中病变位于主动脉弓3例,位于胸降主动脉4例。主动脉穿透性溃疡深度为(6.00±2.02) mm,宽度为(12.07±5.26) mm,长度为(14.71±9.05) mm。手术时间为(90.00±32.53) min。其中3例进行左锁骨下动脉烟囱支架植入。随访1~7个月。随访结果提示主动脉穿透性溃疡均隔绝良好,无各型内漏发生。无1例并发症发生。结论 胸主动脉腔内隔绝术对于主动脉穿透性溃疡安全有效,对于高龄及伴随疾病较多的患者安全性也较高。

Abstract:

ObjectiveTo investigate the indications, therapeutic effects and safety of thoracic endovascular aortic repair for treatment of thoracic penetrating aortic ulcer. MethodsWe retrospectively analyzed clinical data of the patients diagnosed with penetrating thoracic aortic ulcer, who underwent thoracic endovascular aortic repair in our department between October, 2018 and May, 2019. The patients’ age, lesion location and type, comorbidities, surgical methods, operation time, intraoperative blood loss, length of hospital stay, perioperative complications and follow-up data were analyzed. ResultsWe analyzed the clinical data for 7 patients with a mean age of 70.29±7.54 years, including 3 patients with penetrating ulcers in the aortic arch and 4 with ulcers in the descending thoracic aorta. The mean depth of the penetrating aortic ulcers was 6.00±2.02 mm with a mean width of 12.07±5.26 mm and a mean length of 14.71±9.05 mm. The opration were completed smoothly in all the patients with a mean operation time of 90.00±32.53 min. A chimney stent was placed in the left subclavian artery in 3 cases. The patients were followed up for 1 to 7 months, during which the penetrating thoracic aortic ulcers were found to be effectively repaired in all the cases; endoleaks of any types or other complications occurred in none of the cases during the follow-up. ConclusionThoracic endovascular aortic repair is safe and effective for treatment of penetrating thoracic aortic ulcer, and can be safely performed in elderly patients and patients with comorbidities.
