摘要:

目的 研究磁性附着体和球帽附着体在下颌双种植覆盖义齿修复中的临床应用效果。方法 回顾性分析2010年4月至2014年12月在焦作市人民医院口腔科进行下颌种植覆盖义齿修复的患者64例,分别使用磁性附着体(n=32)和球帽附着体(n=32)进行种植覆盖义齿修复,收集两组的咀嚼效率及患者满意度,记录种植体骨边缘情况,3年后随访记录两者的咀嚼效率及患者满意度,测量种植体骨吸收情况,运用SPSS19.0软件对结果进行统计学分析。结果 修复1周后磁性附着体舒适度优于球帽附着体(P=0.019),而球帽附着体固位稳定功能优于磁性附着体(P=0.014),两者的咀嚼效率(P=0.975)及满意度(P=0.787)无统计学差异;修复3年后磁性附着体固位力衰减较球帽附着体小,差异有统计学意义(P=0.000);而种植体边缘骨吸收(P=0.609)及患者咀嚼效率(P=0.933)、满意度(P=0.932)两者无统计学差异。结论 种植覆盖义齿修复3年后,磁性附着体固位功能较球帽附着体稳定,但两者咀嚼效率和总体满意度无明显区别,两种附着体种植体边缘的骨吸收无差异。

Abstract:

Objective To compare the clinical effects of the magnetic attachments and ball attachments for treatment of edentulous mandible with implant-supported overdentures. Methods This retrospective analysis was conducted among 64 patients receiving treatment with implant-supported overdentures for edentulous mandible between April, 2010 and December, 2014. Magnetic attachment was used on the implant-supported overdentures in 32 of the patients and ball attachment was used in the other 32 patients. The masticatory efficiency, the patient’s satisfaction and implant bone absorption were evaluated in the two groups at 1 week and 3 years after the implantation. Results The patients reported significantly better comfort with the magnetic attachment than with the ball attachment (P=0.019), but the latter had better retention and stability at 1 week after dental restoration (P=0.014). No significant differences were found in the masticatory efficiency (P=0.975) or the patients’ satisfaction (P=0.787) between the two groups. At 3 years after dental restoration, the retention force of the magnetic attachment showed significantly less reduction than that of ball attachment (P=0.000), but the bone resorption (P=0.609), masticatory efficiency (P=0.933), or the patients’ satisfaction (P=0.932) did not differ significantly between the two groups. Conclusion The magnetic attachment has more stable retention than ball attachment at 3 years after dental restoration, but the masticatory efficiency and the patients’ satisfaction are comparable between the 2 attachments.
