摘要:

目的     检测HBV相关性肝癌患者CD19+CD24+CD38+调节性B细胞(Bregs细胞)的分布频率及血清白细胞介素10(interleukin-10,IL-10)的水平并分析其临床意义。方法     收集93例HBV相关性肝癌患者、47例HBV相关性肝硬化患者和57例健康体检者的外周血,采用流式细胞术检测CD19+CD24+CD38+Bregs细胞分布频率,化学发光法检测血清IL-10水平,并分析其与疾病分期、肿瘤个数、血管侵犯等临床指标的关系;同时收集20例HBV相关性肝癌患者的癌组织、癌旁组织和远癌组织,采用流式细胞术检测组织中CD19+CD24+CD38+Bregs细胞分布频率,并分析其与肿瘤位置的相关性。结果    HBV相关性肝癌外周血CD19+CD24+CD38+Bregs细胞显著高于肝硬化组和健康对照组,且HBV相关性肝癌晚期(C+D期)患者显著高于早期(0+A期)和中期(B期)患者(P<0.05,P<0.01);HBV相关性肝癌和HBV相关性肝硬化患者血清IL-10水平显著高于健康对照组(P<0.01);HBV相关性肝癌外周血CD19+CD24+CD38+Bregs细胞分布频率和血清IL-10水平与肿瘤个数、是否血管侵犯有密切关系;HBV相关性肝癌患者外周血CD19+CD24+CD38+Bregs细胞分布频率与血清IL10呈正相关(r=0.46, P<0.01);HBV相关性肝癌患者癌组织和癌旁组织CD19+CD24+CD38+Bregs细胞频率显著高于远癌组织(P<0.01)。结论     CD19+CD24+CD38+Bregs细胞可能通过分泌IL-10在HBV相关性肝癌免疫微环境中发挥调节作用,参与肿瘤的发生、发展。

Abstract:

Objective     To investigate the frequency of CD19+CD24+CD38+ regulatory B cells (Bregs) and serum level of interleukin-10 (IL-10) in the HBV associated hepatocellular carcinoma, and analyze their clinical significance.  Methods    The peripheral blood samples were collected from 93 patients with HBV associated hepatocellular carcinoma, 47 patients with HBV associated liver cirrhosis and 57 healthy individuals. The frequency of Bregs subsets (CD19+CD24+CD38+) was detected by flow cytometry, and the level of IL10 was determined by chemoluminescence assay. And the results were analyzed with the disease stage, tumor multiplicity, vascular invasion and other clinical indicators. And the frequencies of the Bregs in the tumor tissues, normal tissues closely adjacent and distal to tumor mass from 20 with HBV associated hepatocellular carcinoma were also determined by flow cytometry, and the relationship of the frequency with the tumor site was analyzed. Results    The frequency of CD19+CD24+CD38+Bregs was significantly higher in the peripheral blood of the patients with HBV associated hepatocellular carcinoma than the patients with liver cirrhosis and healthy controls. And the level was also higher in the patients with advanced carcinoma (stage 0+A) than those at early (stage B) and moderate (stage C+D) stages (P<0.05, P<0.01). The level of IL-10 in the patients with HBV associated hepatocellular carcinoma and HBV associated liver cirrhosis were obviously higher than health controls (P<0.01). The frequency of CD19+CD24+CD38+Bregs and the serum level of IL-10 were closely associated with tumor multiplicity and venous infiltration in the patients with advanced HBV hepatocellular carcinoma. And the 2 indicators were positively correlated with each other (r=0.46, P<0.01). The frequencies of Bregs were remarkably higher in tumor and paracarcinoma tissues than the normal tissues distal to tumor mass (P<0.01).  Conclusion    CD19+CD24+CD38+Bregs may play a regulatory role in immune microenvironment through the secretion of IL-10 in HBV associated hepatocellular carcinoma, and participate in the incidence and development of hepatocellular carcinoma.
