摘要:

目的     通过视功能(visual function,VF)问卷、生存质量(quality of life,QOL)问卷调查、对比敏感度(contrast sensitivity,CS)和对比度视力(contrast visual acuity,CVA),探讨初期皮质性白内障患者的视觉质量。方法     回顾性分析2016年7月至2017年11月于我院眼科白内障门诊就诊的最佳矫正视力≥0.8 100例(160眼)初期皮质性白内障患者的临床资料,正常对照组选择年龄、性别与实验组相匹配的正常人100例(140眼)。行VF、QOL问卷调查评分,检测CS(低频、中频、高频),同时采用视觉灵敏度测量仪测试不同CS(50%、35%、25%)视标的视力,并与正常对照组比较。结果    初期皮质性白内障组的VF值(71.37±5.54)、QOL值(88.38±5.07)均显著低于正常对照组(P<0.05)。初期皮质性白内障组在对比度为50%、35%、25%下CVA[分别为(0.48±0.15)、(0.37±0.11)、(0.25±0.09)]均显著低于正常对照组[分别为(0.83±0.10)、(0.79±0.10)、(0.70±0.12),P<005]。初期皮质性白内障患者中频(6 c/d)CS值(35.05±26.47)、高频(12、18 c/d)CS值[分别为(3.62±3.97)、(0.00±0.00)]显著低于正常对照组中频(6 c/d)CS值(94.75±40.45)、高频(12、18 c/d)CS值[分别为(30.04±17.18)、(10.29±5.69),P<0.05]。初期皮质性白内障组低频CS值(61.67±39.50)与正常对照组低频CS值(107.58±40.25)比较,差异无统计学意义(P>0.05)。结论     初期皮质性白内障患者虽然远视力尚可,但VF、QOL问卷评分及CS、CVA较同年龄段正常人明显异常。VF、QOL问卷和CS、CVA能反映普通视力检查不能辨识的初期皮质性白内障患者的视功能差异。

Abstract:

Objective     To assess the visual quality in the patients with early cortical cataract by using visual function (VF) and quality of life (QOL) questionnaires, contrast sensitivity (CS), and contrast visual acuity (CVA). Methods     A retrospective study was performed on 100 out-patients with early cortical cataracts (160 eyes, with best corrected visual acuity ≥0.8) admitted in our hospital from July 2016 to November 2017. And 100 healthy participates (140 eyes) with matched age and sex served as control. VF and QOL questionnaires, CS test (low, medium and high frequencies) and different CVA test (with contrast sensitivity of 50%, 35% and 25%) were used, and the results were compared between the 2 groups. Results     The scores of VF and QOL questionnaires were 71.37±5.54 and 88.38±5.07 respectively, both significantly lower than those of control group (P<0.05). The average CVA at 50%, 35% and 25% were 0.48±0.15, 0.37±0.11 and 0.25±0.09, respectively, in the cataracts group, obviously lower than those of control group (0.83±0.10, 0.79±0.10 and 0.70±0.12, P<0.05). For the cataract group, the CS value at average medium frequency (6  c/d) was 35.05±26.47, and the values at the average high frequency (12  c/d and 18  c/d) were 3.62±3.97 and 0.00±0.00, and all these values were notably lower than those of control group (6  c/d: 94.75±40.45, 12  c/d: 30.04±17.18, 18  c/d: 10.29±5.69, all P<0.05). As for the low frequency CS, there was no differences between the groups (61.67±39.50 vs 107.58±40.25, P>0.05). Conclusion    Although the patients with early cortical cataract have normal visual acuity, but their scores of VF and QOL questionnaires, as well as the results of CS and CVA were obviously abnormal when compared with the healthy individuals at the same age period. The questionnaires and tests can reflect the visual function loss in the patients which can’t be identified by visual acuity test.
