摘要:

目的     了解慢性化脓性中耳炎耳分泌物病原微生物类型及致病菌的药物敏感性,指导临床合理用药。方法     收集2013年1月至2015年3月慢性化脓性中耳炎住院患者(纳入患者555例,共计571耳,其中男性272耳,女性299耳)耳分泌物,经细菌培养和药敏试验,进行病原微生物分离率的排序,检测致病菌的药物敏感度。结果     571耳中培养有微生物生长379耳(占66.37%),其中细菌334耳(占88.13%)、真菌47耳(占12.40%)。细菌检出率依次为凝固酶阴性葡萄球菌191耳(占57.19%)、金黄色葡萄球菌58耳(占17.37%)、铜绿假单胞菌31耳(占9.28%)。药物敏感性因菌种而异。结论     慢性化脓性中耳炎活动期患者耳分泌物以凝固酶阴性葡萄球菌、金黄色葡萄球菌、铜绿假单胞菌为主。药敏试验对临床用药有指导作用。

Abstract:

Objective      To analyze the pathogenic distribution and antibiotic susceptibility of chronic suppurative otitis media (CSOM) in order to guide  rational drug application. Methods     Middle ear pus samples were collected from 555 hospitalized patients (571 ears)  with chronic suppurative otitis media in our department from January 2013 to March 2015 and cultured for bacteria and antibiotic susceptibility test.  Results     There were 379 strains (66.37%) of microbes isolated from 571 ears, including 334 ears of bacteria (88.13%) and 47 ears (12.40%) of fungi. The most frequently isolated pathogens were Coagulase-negative Staphylococcus (191 ears, 42.51%), Staphylococcus aureus (58 ears, 17.37%), and Pseudomonas aeruginosa (31 ears, 9.28%). The antibiotic sensitivity was variable among the isolated bacterial strains. Conclusion      Coagulase-negative Staphylococcus, Staphylococcus aureus and Pseudomonas aeruginosa are the main pathogenic bacteria in chronic suppurative otitis media. The susceptibility test are beneficial for choosing appropriate antibiotics.
