摘要:

类鼻疽是一种由类鼻疽伯克霍尔德菌导致的热带医学疾病,其临床表现多样,也有“似百样病”之称,其病原已被美国CDC列为生物恐怖剂。类鼻疽在中国的海南、广西以及福建等地都有过报道,但由于其误诊率高,也未被列为法定报告的传染病,对类鼻疽的研究并未得以重视。类鼻疽菌对多种抗生素具有天然的耐药性,同时其基因组变异迅速,能产生新的耐药特性,造成的感染极易复发,治疗困难。目前临床上对类鼻疽的耐药检测还未建立统一的检测指标和判断标准,国内也还没有规范的类鼻疽抗生素用药指南。我们总结了中国2006-2014年公开发表文献中的类鼻疽临床用药及耐药结果,结合本室的类鼻疽临床资源库数据,对中国类鼻疽菌的耐药谱进行统计分析,总结用药规律,以期寻找合适的用药方案;同时,还分析了类鼻疽菌的可能耐药机制,探讨了类鼻疽耐药的深层次原因,以期为后续类鼻疽的治疗及用药指南的制定提供理论指导和临床借鉴参考。

Abstract:

Melioidosis is a tropical disease caused by Burkholderia pseudomallei, and is known as “like a hundred of different diseases” because of its variety of clinical manifestations. It is widely distributed in China, including Hainan, Guangxi and Fujian provinces, with high infectivity, high mortality and high relapse. So far, there is no standard therapeutics for melioidosis in China, and standard drug resistance test has not been established yet. Therefore, we summarize the melioidosis cases and related drug resistance data published in Chinese literatures from 2006 to 2014 and related cases that we collected. The drug resistance spectrum of Burkholderia pseudomallei and possible drug resistance mechanisms are further analyzed, which can provide theoretical direction and clinical reference for therapy and medication guide of melioidosis.
