摘要:

目的      对中国人下颌第一前磨牙牙根及根管形态的研究作一系统性的回顾分析。      方法      通过国内外数据库检索所有与中国人下颌第一前磨牙牙根及根管形态相关的文献以及病例报道,记录下颌第一前磨牙牙根数目、根管数目、根管分型、根尖孔数目和根尖孔位置。      结果      1 324颗研究牙根数目的下颌第一前磨牙中,单个牙根发生率占93.1%,2个牙根占6.6%,3个牙根占0.3%,其中未发现4个牙根的存在;5 745颗研究根管数目的下颌第一前磨牙中,单根管发生率占75.2%,多根管占24.8%;4 650颗研究根管分型的下颌第一前磨牙中,Ⅰ型根管发生率占75.1%,Ⅱ型占2.0%,Ⅲ型占1.7%,Ⅳ型占5.5%,Ⅴ型占12.5%,Ⅵ型占0.1%,Ⅶ型<0.1%,Ⅷ型占0.5%,其他根管类型占2.6%;478颗研究根尖孔数目的下颌第一前磨牙中,只有1个根尖孔的占63.2%,有2个根尖孔的占25.7%,有3个根尖孔的占9.0%,有4个根尖孔的占2.1%;771颗研究根尖孔开口位置的下颌第一前磨牙中,包含946个根尖孔,其中根尖孔位于根尖顶端的占56.6%,位于根尖侧方的占43.4%。      结论      中国人下颌第一前磨牙以单牙根单根管为主,两牙根两根管常见,两根管中主要以Ⅳ型和Ⅴ型根管为主,其他根管分型发生率较低,但包含根管类型较多。此外,下颌第一前磨牙根尖孔数目变化大,约占一半的根尖孔位于根尖侧方。

Abstract:

Objective      To perform a systematic review of studies on root and root canal morphology in mandibular first premolars of Chinese.       Methods      An exhaustive search was undertaken to identify associated literatures and case reports of root and root canal morphology in mandibular first premolars of Chinese through domestic and foreign database. The number of roots, number of root canals, root canal configurations, number of apical foramina and location of apical foramina were recorded.       Results      In the 1 324 mandibular first premolars for investigating roots, cases of single root accounted for 93.1%, cases of 2 roots accounted for 6.6%, cases of 3 roots accounted for 0.3%, and no case of 4 roots was found. In the 5 745 mandibular first premolars for investigating root canals, cases of single root canal accounted for 75.2%, and cases of multiple root canals accounted for 24.8%. Based on Vertucci classification of 4 650 mandibular first premolars, it was found that cases of type Ⅰ accounted for 75.1%, cases of type Ⅱ accounted for 2.0%, cases of type Ⅲ accounted for 1.7%, cases of type Ⅳ accounted for 5.5%, cases of type Ⅴ accounted for 12.5%, cases of type Ⅵ accounted for 0.1%, cases of type Ⅶ accounted for less than 0.1%, cases of type Ⅷ accounted for 0.5%, and cases of other root canals accounted for 2.6%. In 478 mandibular first premolars for investigating apical foramina, cases of single apical foramen accounted for 63.2%, cases of 2 apical foramina accounted for 25.7%, cases of 3 apical foramina accounted for 9.0%, and cases of 4 apical foramina accounted for 2.1%. In 771 mandibular first premolars containing 946 apical foramina, cases of top apical foramina accounted for 56.6%, and cases of lateral apical foramina accounted for 43.4%.        Conclusion      Most mandibular first premolars of Chinese feature single root and single root canal, and cases of 2 roots and 2 root canals (mainly type Ⅳ and type Ⅴ) are common. There is low incidence but great variety of other root canal classifications. In addition, the number of apical foramina in mandibular first premolars changes greatly, and nearly a half of apical foramina is located laterally.
