摘要:
目的      探讨血管细胞黏附分子-1(vascular cell adhesion molecule-1,VCAM-1)和巨噬细胞游走抑制因子(macrophage migration inhibitory factor,MIF)对糖尿病患者的临床意义及其在糖尿病肾病诊断过程中的作用。      方法       选取2型糖尿病患者126例和20例健康体检者(健康对照组),测量VCAM-1、MIF、尿肌酐、血清肌酐等指标,估算糖尿病患者的肾小球滤过率(eGFR)。比较不同研究对象的VCAM-1与MIF水平,采用Pearson相关分析探讨VCAM-1及MIF与尿白蛋白/肌酐、eGFR的关联。      结果      糖尿病患者的VCAM-1、MIF水平明显高于健康对照组,尿白蛋白/肌酐越高,VCAM-1、MIF越高(P<0.05);VCAM-1和MIF水平与eGFR均呈明显的负相关关系(r2=0.240 5, r2=0.516 2, P<0.05);VCAM-1和MIF水平与尿白蛋白/肌酐值均呈正相关关系(r2=0.069 5, r2=0.059 5, P<0.05);VCAM-1和MIF水平与eGFR变化均呈负相关(r2=0.048 2, r2=0.071 7,P<0.05)。      结论      糖尿病及糖尿病肾病患者血浆中VCAM-1及MIF水平升高,对糖尿病肾病诊断及病情判断具有一定的参考价值。
Abstract:
Objective       To determine the clinical significance of vascular cell adhesion molecule-1 (VCAM-1) and macrophage migration inhibitory factor (MIF) in diabetic patients and determine their role in the diagnosis of diabetic nephropathy.       Methods      A total of 126 patients of type 2 diabetes in our department from January 2010 to January 2012 were recruited in this study, and 20 healthy subjects were subjected as normal control. Their serum levels of VCAM-1, MIF and creatinine, urine creatinine and estimated glomerular filtration rate (eGFR) in diabetic patients were measured and calculated. VCAM-1 and MIF levels from different groups were compared and the correlation of MIF and VCAM-1 with urinary albumin/creatinine and eGFR was analyzed by Pearson correlation analysis.       Results      Serum levels of VCAM-1 and MIF were significantly higher in diabetic patients than the healthy control (P<0.05), and the higher the urinary albumin/creatinine, the higher VCAM-1 and MIF levels were (P<0.05). Serum levels of VCAM-1 and MIF showed a significant negative correlation with eGFR (r2=0.240 5, r2=0.516 2,P<0.05), a positive correlation with urinary albumin/creatinine (r2=0.069 5, r2=0.059 5, P<0.05); The changes of VCAM-1 and MIF levels compared with the levels in 1 year later showed a negative correlation with changes in eGFR (r2=0.048 2, r2=0.071 7,P<0.05).       Conclusion      Serum levels of VCAM-1 and MIF are elevated in the patients with diabetes or diabetic nephropathy, and the levels provide a certain reference value for the diagnosis of diabetes and kidney disease.
