摘要:
目的      探讨核转录因子过氧化物酶体增殖物激活受体-γ(peroxisome proliferator-activated receptor gamma, PPAR-γ)在猪颈动脉支架植入后的表达及对血管平滑肌细胞(vascular smooth muscle cell,VSMC)表型转化的影响。      方法      小型猪12只,为3组:①正常对照组(4只);②支架植入组(4只);③罗格列酮(ROSI)治疗支架组(4只)。应用血管造影观察血管形态;应用免疫组织化学方法观察支架段血管病理组织形态及PPAR-γ的表达;Western  blot检测支架段血管中PPAR-γ、平滑肌22α (smooth muscle 22 alpha,SM22α)蛋白的表达并分析其与血管重构过程中 VSMC表型转化的关系。      结果      ①血管造影显示3个月后支架植入组狭窄程度较ROSI治疗支架组显著[分别为(1.46±0.14)、(0.91±0.13)mm,(P<0.05)]。②HE染色结果发现支架植入组血管内膜平滑肌增生较ROSI治疗支架组明显;免疫组化示支架植入组血管中PPAR-γ的表达评分较正常对照组、ROSI治疗支架组低[分别为(0.90±0.22)、(5.85±0.29)、(5.04±0.46),P<0.05,ROSI治疗支架组与正常对照组间PPAR-γ表达无显著差异(P>0.05)]。③Western  blot检测结果显示:支架植入组PPAR-γ/SM22α的表达水平值低于正常对照组[分别为(3.12±1.42)%/(16.72±5.34)%、(19.87±5.33)%/(68.85±9.73)%, P<0.05],VSMC表型由收缩型向合成型转化;ROSI治疗支架组PPAR-γ/SM22α的表达[(23.17±4.23)%/(58.63±12.81)%]高于支架植入组(P<0.05),VSMC表型由合成型向收缩型转化;ROSI治疗支架组PPAR-γ/SM22α表达水平与正常对照组相比无显著差异(P>0.05)。      结论      PPAR-γ参与了支架植入术后VSMC表型转化过程,PPAR-γ可能是治疗支架植入术后再狭窄的潜在靶点之一。
Abstract:
Objective      To study porcine peroxisome proliferator activated receptor gamma (PPAR-γ) expression changes after carotid artery stent implantation and its influence on the vascular smooth muscle phenotype.       Methods      Twelve miniature pigs were randomly divided into 3 groups, normal control group (control group, n=4), stent implantation group (stent group, n=4), and stent implantation combined with rosiglitazone (ROSI) treatment group (ROSI group, n=4). Angiography was used to observe vascular morphology. Immunohistochemical assay was used to observe the vascular pathological morphology and PPAR-γ expression. Western blotting was used to test the expression changes of PPAR-γ and smooth muscle 22 alpha (SM22α) in blood vessels with stents and analyze their relationship with vascular smooth muscle phenotype transformation of vascular remodeling.       Results      Angiography showed that the extent of vessel stenosis in the stent group was higher than that in the ROSI group after 3 months (1.46 mm±0.14 mm vs  0.91 mm±0.13 mm, P<0.05). The results of HE staining found that in stent group, the intimal proliferation of smooth muscle was higher than that in ROSI group. Immunohistochemical assay results showed the expression score of PPAR-γ in the stent group was lower than those in the control group and ROSI group (0.90±0.22 vs 5.85±0.29 vs  5.04±0.46, P<0.05), while the ROSI group and control group had no significant difference (P>0.05). Western blot results showed that the expression of PPAR-γ/SM22α of the stent group was lower than that of the control group [(3.12±1.42)%/(16.72±5.34)% vs  (19.87±5.33)%/(68.85±9.73)%, P<0.05]. Phenotype of vascular smooth muscle of the stent group transformed from contractile type to synthetic type. Expression of PPAR-γ/SM22α in the ROSI group [(23.17±4.23)%/(58.63±12.81)%] was higher than that of the stent group (P<0.05). Phenotype of vascular smooth muscle of the ROSI group transformed from synthetic type to contractile type. No significant difference was observed in PPAR-γ/SM22α expression between the ROSI group and the control group.       Conclusion      PPAR-γ participates in the process of vascular smooth muscle phenotype conversion after stent implantation, and thus may be one potential target for the treatment of restenosis after stent implantation.
