摘要:
目的      探讨患喘息性疾病的婴幼儿血清白介素-4(IL-4)、干扰素-γ(IFN-γ)及尿白三烯E4(LTE4)浓度的变化及其意义。      方法      采用竞争性酶联免疫吸附法(ELISA)检测22例急性期、恢复期喘息性疾病患儿(喘息组),20例急性期非喘息的下呼吸道感染患儿(非喘息组),20例健康体检儿(正常对照组)血清IL-4、IFN-γ及尿LTE4水平,分析IL-4与IFN-γ的比值,分析IL-4、IFN-γ与尿LTE4的相关性。      结果      喘息组急性期血清IL-4水平较非喘息组急性期及正常对照组均明显升高(P<0.05,0.01);血清IFN-γ水平明显降低(P<0.01);尿LTE4水平较正常对照组明显升高(P<0.01),但与非喘息组急性期比较差异无统计学意义(P>0.05);喘息组恢复期血清IL-4水平较急性期明显下降(P<0.01),但仍高于正常对照组(P<0.01);血清IFN-γ水平明显升高(P<0.01),但仍低于正常对照组(P<0.01);尿LTE4水平明显降低(P<0.01),但仍高于正常对照组(P<0.01)。喘息组急性期血清IL-4/IFN-γ比值较非喘息组急性期及正常对照组均明显升高(F=15.86,P<0.01);恢复期血清IL-4/IFN-γ比值较急性期明显下降(t=4.14,P<0.01),但仍高于正常对照组,且有统计学差异(t=5.94,P<0.01)。喘息组急性期尿LTE4水平与血清IL-4水平呈正相关(r=0.82,P<0.01),与血清IFN-γ水平呈负相关(r=-0.81,P<0.01)。      结论      IL-4、IFN-γ及LTE4可能共同参与婴幼儿喘息性疾病的气道炎症反应,喘息患儿存在Th1/Th2免疫失衡。
Abstract:
Objective      To investigate the changes in serum levels of interleukin-4 (IL-4), interferon-γ(IFN-γ)and urinary level of leukotriene E4(LTE4) in infants with asthmatic diseases and their significances.       Methods      Serum levels of IL-4 and IFN-γ and urinary levels of LTE4 were measured by competitive ELISA in 22 infants with acute and convalescence asthmatic diseases (asthmatic group), 20 infants with acute non-asthmatic diseases but with respiratory tract infection (non-asthmatic group), and 20 healthy infants (normal control group). The ratio of IL-4 to IFN-γ and the correlations between urinary LTE4 and serum IL-4, and between urinary LTE4 and serum IFN-γ were analyzed.       Results      The serum levels of IL-4 in acute asthma period was obviously higher than that of acute non-asthma and normal control(P<0.05, 0.01). The serum levels of IFN-γ in acute asthma period was significantly lower than those of acute non-asthma and normal control(P<0.01). The urinary levels of LTE4 in acute asthma period was higher than normal control group(P<0.01), but were not significantly different from acute non-asthma (P>0.05). In remission/convalescence, the serum levels of IL-4 were lower than those of acute period (P<0.01), but were still higher than normal control group (P<0.01). The serum levels of IFN-γ were higher than those of acute period (P<0.01), but were still lower than normal control group (P<0.01). The urinary levels of LTE4 were obviously lower than those of acute period (P<0.01), but were still higher than normal control (P<0.01). The ratio of IL-4 to IFN-γ in acute asthma period was higher than that of acute non-asthma and normal control group(F=15.86,P<0.01).In remission, the ratio of IL-4 to IFN-γ was lower than that in acute period (t=4.14, P<0.01), but was still higher than normal control group (P<0.01). There was positively correlation between urinary LTE4 and serum IL-4 levels in the acute stage in asthmatic group(r=0.82, P<0.01). There was negative correlation between urinary LTE4 and serum IFN-γ levels in the acute stage in asthmatic group(r=-0.81, P<0.01).       Conclusion      IL-4, IFN-γ and LTE4 may all be involved in the pathogenesis of respiratory tract inflammation in infant with asthmatic diseases. There are immune imbalance of Th1/Th2 in infant with wheezing.
