目的     探讨人8-羟基鸟嘌呤DNA糖苷酶(human 8- hydroxyguanine deoxyribonucleic acid glycosidase,hOGG1)在小细胞肺癌中的表达特点及其与患者预后的关系。     方法      应用免疫组化检测71例小细胞肺癌组织和30例正常肺组织中hOGG1的表达,按细胞阳性百分数及染色强度将其分为低表达组和高表达组,并分析hOGG1在小细胞肺癌表达与临床病理及患者预后的关系。     结果      正常肺组织与小细胞肺癌组织中均有hOGG1表达,但正常肺组织以细胞质表达为主(60.0%),而小细胞肺癌组织以核质共表达为主(49.3%),两者亚细胞定位有明显差异(P<0.01);在hOGG1表达高低方面,正常肺组织100%为低表达,而小细胞肺癌组织78.9%为高表达,两者间也存在明显差异(P<0.01);hOGG1的表达与患者的性别、肿瘤分期、吸烟指数、有无淋巴结转移及有无胸水无明显关系(P>0.05);Kaplan-Meier分析显示hOGG1的表达强弱与患者的预后明显相关(P<0.05),hOGG1低表达组中位生存时间为23个月,hOGG1高表达组中位生存时间为13个月。COX比例风险模型分析结果提示hOGG1是影响患者生存的独立预后因素(P=0.004)。     结论      hOGG1蛋白核质共表达为主的高表达与小细胞肺癌的发生及发展有关,hOGG1蛋白表达增强可能是小细胞肺癌患者预后不良的指标之一。
Abstract:
Objective       To study the expression of human 8-hydroxyguanine deoxyribonucleic acid glycosidase 1(hOGG1)in small cell lung cancer and its correlation with the prognosis of small cell lung cancer patients.     Methods       Expression of hOGG1 was detected by immunohistochemistry in 71 small cell lung cancer tissue samples and 30 normal lung tissue samples. Small cell lung cancer were divided into high expression group and low expression group according to their positive percentage and staining intensity. Correlation of the hOGG1 expression in small cell lung cancer tissue samples with the clinicopathology and prognosis of small cell lung cancer was analyzed.      Results       The hOGG1 was expressed both in small cell lung cancer tissue samples and in normal lung tissue samples. The expression level of hOGG1 was higher in cytoplasm of normal lung tissue samples than in cytoplasm of small cell lung cancer tissue samples(60.0% vs 49.3%, P<0.01). The hOGG1 was none over-expressed in normal lung tissue samples but in 78.9% of small cell lung cancer tissue samples(P<0.01). The hOGG1 expression was not significantly correlated with gender, tumor staging, smoking index number, lymphatic metastasis or hydrothorax(P>0.05). Kaplan-Meier analysis showed that the hOGG1 expression level was significantly correlated with the prognosis of small cell lung cancer patients(P<0.05). The median survival time of small cell lung cancer patients with a low hOGG1 expression level and a high hOGG1 expression level was 23 months and 13 months, respectively.COX multivariate proportional hazards model analysis indicated that hOGG1 was an independent factor for the prognosis of small cell lung cancer patients(P=0.004).      Conclusion       The high hOGG1 expression level in cytoplasm is correlated with the occurrence and progress of small cell lung cancer and is one of the poor prognosis indicators of small cell lung cancer patients.
