目的    探讨CD68+CD163+巨噬细胞在食管癌组织浸润及其对预后的影响。    方法    应用免疫组化方法检测CD68+、CD163+巨噬细胞在食管癌组织的浸润,比较CD68、CD163阳性巨噬细胞浸润密度及对食管癌患者预后的影响。    结果    食管癌组织CD68、CD163阳性巨噬细胞浸润密度较癌旁正常组织明显增多(t=21.379, P<0.01; t=21.712, P<0.01)。 CD68+、CD163+巨噬细胞浸润密度与患者生存时间均呈负相关性(r=-0.281, P<0.01; r=-0.344, P<0.01)。CD68+、CD163+巨噬细胞浸润高密度组5年生存率低于低密度组,分别为30.5% vs 56.9%, 27.1% vs 60.3% (P<0.05),但COX风险因素分析CD68+巨噬细胞不是独立的预后因素(P>0.05),而CD163+巨噬细胞是独立的预后因素(P<0.01)。    结论    CD68+、CD163+巨噬细胞在食管癌组织中具有较高的浸润密度,CD163+巨噬细胞浸润密度与患者预后具有密切相关性。
Abstract:
Objective     To investigate the infiltrations of CD68- and CD163-positive macrophages in esophageal cancer tissue and their impacts on prognosis.     Methods     One hundred and seventeen esophageal cancer specimens were resected from January 1, 2004 to May 30, 2005 in our hospital and Henan Tumor Hospital. The infiltration densities of CD68- and CD163-positive macrophages in each esophageal cancer specimen were measured by immunohistochemical staining, and the specimens were divided into high-density and low-density groups for CD68-positive macrophages and CD163-positive macrophages. Homogeneity of variance test and student’s t-test, Spearman’s test, Kaplan-Meier survival analysis, and Cox regression analysis were used to compare the infiltration densities of CD68- and CD163-positive macrophages in esophageal cancer tissue with those in adjacent normal tissue and evaluate the impacts of infiltration densities of CD68- and CD163-positive macrophages on prognosis of esophageal cancer.     Results    The infiltration densities of CD68- and CD163-positive macrophages were significantly higher in esophageal cancer tissue than in adjacent normal tissue (t=21.379, P<0.01; t=21.712, P<0.01). The infiltration densities of CD68- and CD163-positive macrophages were negatively correlated with the survival times of patients (r=-0.281, P<0.01; r=-0.344, P<0.01). The 5-year survival rate was lower in high-density groups than in low-density groups for both CD68-positive macrophages and CD163-positive macrophages (30.5% vs 56.9%, P<0.05; 27.1% vs 60.3%, P<0.05). The infiltration density of CD68-positive macrophages was not an independent prognostic factor (P>0.05), but that of CD163-positive macrophages was (P<0.05), according to Cox regression analysis.     Conclusion    CD68- and CD163-positive macrophages show relatively high infiltration densities in esophageal cancer tissue. The infiltration density of CD163-positive macrophages is negatively correlated with the prognosis of esophageal cancer.
