摘要:
目的   观察新诊断2型糖尿病患者多因素干预与心肌缺血的关系。   方法   新诊断2型糖尿病患者120例经血糖、血脂、血压强化干预8年后,采用腺苷负荷下心肌核素显像(myocardial perfusion imaging,MPI)评估心肌缺血,统计分析多因素控制达标情况与MPI阳性率之间的关系。   结果   本组患者心肌缺血阳性率为25%,Logistic分析示有胸闷史(OR=49.963,95%CI=9.525~262.067,P=0.000),高血压超过130/85 mmHg(OR=4.684,95%CI=1.064~20.239,P=0.032),总胆固醇超过4.66 mmol/L(OR=7.297,95%CI=1.247~42.691,P=0.027),糖化血红蛋白超过7%(OR=4.640,95%CI=1.064~20.239,P=0.041)4项因素均为发生糖尿病性心肌缺血的高危因素。   结论   4项危险因素控制不良的2型糖尿病患者心肌缺血发生率高。全面、严格控制危险因素,尽可能提高治疗达标率才可能减少心肌缺血的发生。
Abstract:
Objective   To observe the correlation between intervention of multiple factors and myocardial ischemia in patients with newly diagnosed type 2 diabetes.    Methods   Myocardial ischemia in patients with newly diagnosed type 2 diabetes (duration≤1 year) was assessed 8 years after intensified intervention of their blood sugar, lipid and pressure using adenosine load myocardial nuclide perfusion image. Correlation between multiple factor analysis results using SPSS 13.0 and positive rate of adenosine load myocardial nuclide perfusion image was observed.    Results   The positive rate of myocardial ischemia in patients included in this study was 25%. Logistic analysis showed that history of chest distress (OR=49.963,95%CI=9.525 to 262.067,P=0.000), blood pressure>130/85 mmHg (OR=4.684,95%CI=1.064 to 20.239, P=0.032), total cholesterol>4.66 mmol/L (OR=7.297, 95%CI=1.247 to 42.691, P=0.027) and mean glycosylated hemoglobin >7% (OR=4.640,95%CI=1.064 to 20.239, P=0.041) were the high risk factors for myocardial ischemia in patients with type 2 diabetes.    Conclusion   Poor control of the 4 factors is the high risk factor for myocardial ischemia in patients with type 2 diabetes. Overall and effective control of such risk factors can decrease the incidence of myocardial ischemia and improve its treatment.
