目的  研究伴有高危型人乳头瘤病毒(highrisk human papillomavirus, hr-HPV)感染及无hr-HPV感染的宫颈癌(cervical cancer, CC)、宫颈上皮内瘤变(cervical intraepithelial neoplasia, CIN)及宫颈炎中p16INK4a蛋白表达水平的变化及意义,探讨宫颈癌及癌前病变中p16INK4a 蛋白表达与HPV感染的相关性。  方法  应用免疫组化方法检测126例宫颈疾病(CC 40例,CIN 67例和宫颈炎19例)组织中p16INK4a蛋白的表达,其中,hr-HPV(+)宫颈疾病89例(CC 32例,CIN 48例和宫颈炎9例),hr-HPV(-)宫颈疾病37例(CC 8例,CIN 19例和宫颈炎10例),进行半定量分析及统计学检验。  结果  40例CC中,p16INK4a全部阳性表达;48例hrHPV(+)和19例hrHPV(-)CIN中,p16INK4a阳性表达分别为39例和12例;9例hrHPV(+)和10例hrHPV(-)宫颈炎组织中,p16INK4a阳性表达均为4例。hr-HPV(+)或hr-HPV(-)宫颈炎、宫颈上皮内瘤变和宫颈癌组织中p16INK4a蛋白表达水平均依次升高。CC和CIN中p16INK4a蛋白表达与hr-HPV感染相互间无显著相关性(P>0.05)。  结论  细胞周期抑制蛋白p16INK4a在CC及癌前病变中过表达,且与hr-HPV感染无关,提示p16INK4a蛋白在宫颈癌的发生、发展中所起的作用和作用机制可能与其在其他恶性肿瘤中不同。检测p16INK4a蛋白表达有助于CC的早期诊断。
Abstract:
Objective  To study whether p16INK4a protein expressions in cervical cancer (CC), cervical intraepithelial neoplasia (CIN) and cervicitis are associated with highrisk human papillomavirus (hr-HPV) infection.   Methods  The expressions of p16INK4aprotein were detected in 126 samples of cervical lesions (40 CC, 67 CIN, 19 cervicitis) by immunohistochemical staining, among which 89 cervical lesions were hr-HPV(+) (32 CC, 48 CIN, 9 cervicitis) and 37 hr-HPV(-) (8 CC, 19 CIN, 10 cervicitis).   Results  p16INK4aprotein was positive in all of CC samples. The positive expression of p16INK4a was in 39 hr-HPV(+) samples and 12 hr-HPV(-) CIN samples, with no significant difference between each other. The positive expression of p16INK4a was in 4 hr-HPV(+) and 4 hr-HPV(-) cervicitis samples. The p16INK4a expression increased in the samples of cervicitis, CIN to CC in turn, and the significant difference was observed among them regardless of hr-HPV infection. Pearson correlation analysis revealed no correlation between hr-HPV infection and p16INK4a protein expressions in CC and CIN (P>0.05).   Conclusion  Overexpression of INK4a gene indicates the potential of CC development. Moreover, there is no correlation between the overexpression of p16INK4a protein and hr-HPV state in cervical lesions. Therefore, it is likely that p16INK4a protein expression is not induced by the same carcinogenic mechanisms of CC and other malignant tumors. Overexpression of INK4a gene helps in the diagnosis of cervical cancer.
