摘要:
目的 探索药物预处理透析器的临床应用价值以及使用还原型谷胱甘肽的最佳方案。  方法  2006年1-6月我院住院和门诊接受维持性血液透析患者51例,分为口服还原型谷胱甘肽组、透析前还原型谷胱甘肽预处理组、口服+预处理双重干预组进行6周治疗。同时设立健康对照组。治疗6周后检测血中超敏C反应蛋白(hs-CRP)、白介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、晚期氧化蛋白产物(AOPP)、丙二醛(MDA)的变化并进行分析。  结果 无论采用还原型谷胱甘肽口服还是预处理透析器,均能有效地减少患者血中CRP、IL-6、TNF-α、AOPP、MDA水平。而同时采用这两种措施干预则疗效更佳。  结论  使用还原型谷胱甘肽预处理透析器能够有效改善患者的氧化应激和微炎症状态,而且与口服治疗的疗效相当。而同时采用两种方式干预疗效好于单独使用,且观察期间无明显不良反应。
Abstract:
Objective To compare the effects of reduced glutathione by predisposing dialyser and/or taken orally on oxidative stress and microinflammatory state in patient with maintenance hemodialysis (MHD).   Methods Fifty-one MHD patients visiting our hospital from January to June 2006, were randomly assigned to receive oral reduced glutathione, predisposing dialyser with reduced glutathione or both for 6 weeks. Ten healthy subjects from out-patient clinic for physical examination served as control. The levels of malondialdehyde (MDA), advanced oxidation protein products (AOPP), high-sensitivity C-reactive protein (hs-CRP), interleukin (IL-6), tumor necrosis factor (TNF-α) in plasma were measured before and after the treatment.   Results The levels of hs-CRP, IL-6, TNF-α, AOPP, MDA 6 weeks after treatment of either therapy were all lower than those before treatment. The combined therapy of reduced glutathione achieved the best therapeutic effect.   Conclusion Either reduced glutathione taken orally or by predisposing improves oxidative stress and microinflammatory state in MHD patients, but their combination achieves better therapeutic effect, without apparent side effect.
