目的    探讨转甲状腺素蛋白(transthyretin, TTR)在肥胖患者中的变化以及与血脂、胰岛素敏感性和胰岛β细胞功能的关系。    方法    在流行病学调查中对所调查对象测量人体测量学指标,进行75 g口服葡萄糖耐量试验,测定血脂、游离脂肪酸、血糖、胰岛素和TTR等。将调查对象分为肥胖组和非肥胖组。    结果    ①肥胖与非肥胖相比较,TTR的水平显著高于对照组[分别为(351.9±48.6) mg/L和(327.1±58.5) mg/L,P<0.01]。②在所有受试者中,TTR与TG、TC、LDL、FFA、WHR和BMI显著正相关(r值分别为0.535, 0.369, O.298, 0.174, 0.393, 0.282, P<0.01),和HDL、HOMA-IS和DI显著负相关(r值分别为-0.24,-0.173,-0.157,P<0.01)。③多元逐步回归分析显示,影响TTR的主要因素是TG、TC、WHR和年龄。    结论    在非急性应激状态下,TTR长期地升高与肥胖,尤其是中心型肥胖密切相关;可以从一定程度上反映脂质代谢紊乱和胰岛素敏感性;可能也与胰岛β细胞功能的损伤有关。
Abstract:
Objective    To explore the transthyretin (TTR) level in obesity and its relationship with blood lipid, insulin sensitivity and β-cell function.     Methods    Somatometry and seventy-five gram oral glucose tolerance test were conducted, then blood lipid profile, free fatty acid, blood glucose, insulin and TTR were assayed. The included subjects were divided into obesity and non-obesity according to international standard.      Results    The TTR level in obesity was significantly higher than that in non-obesity[(351.9±48.6) mg/L vs (327.1±58.5)mg/L, P<0.01]. There were obvious positive correlations of TTR with TG, TC, LDL, FFA, WHR, BMI (r=0.535, 0.369, 0.298, 0.174, 0.393, 0.282 respectively, P<0.01), and negative correlations of TTR with HDL, HOMA-IS, DI (r=-0.24, -0.173, -0.157, P<0.01). Multielement stepwise regression analysis suggested that TG, TC, WHR and age were major factors to regulate the TTR level.     Conclusion    Long-term increase of TTR is closely correlated with obesity, especially central obesity, reflecting metabolic disorder of lipid and insulin sensitivity to some extent, and being related to injury of islet β-cell function.
