造血干细胞移植(hematopoietic stem cell transplantation,HSCT)已被广泛用于治疗多种恶性和非恶性血液系统疾病,其适应证也在逐步扩大。由于移植前预处理所致的粒细胞缺乏症、移植物抗宿主病(graft-versus-host disease,GVHD)的预防及治疗,以及广谱抗菌药物的使用等,侵袭性真菌病(invasive fungal disease,IFD)成为造血干细胞移植受者感染相关死亡的主要原因之一[1]。血液病患者IFD的病原菌主要为酵母菌及霉菌,近年来由于氟康唑、泊沙康唑等预防治疗药物的广泛应用,侵袭性念珠菌病(invasive candidiasis,IC)的比例有所下降,而侵袭性霉菌病(invasive mold disease,IMD), 尤其是侵袭性曲霉菌病(invasive aspergillosis,IA)的感染日益受到重视[2]。依据IFD的主要病原菌谱的不同,其预防也应有所不同。基于多个前瞻性临床研究,新的抗真菌治疗药物及方案被引入临床实践[3]。我国侵袭性真菌感染工作组推荐分层进行抗真菌的预防治疗、经验性治疗、诊断驱动治疗及目标治疗[4]。依据IFD的独立危险因素可将造血干细胞移植患者进行危险分层,对于具有IFD高危因素的患者,经验性抗真菌治疗可有效降低确诊/临床诊断IFD的发生,诊断驱动治疗则更适合低风险的患者。依据危险分层制定抗真菌防治方案,可提高抗真菌药物的使用效率,也可避免低危患者过度治疗带来的脏器负担和经济负担。然而,抗真菌预防治疗以及新型抗真菌药物的广泛使用可能导致罕见菌株及耐药菌株的出现,甚至发生突破性侵袭性真菌病(breakthrough invasive fungal disease,bIFD)[1, 5]。分析bIFD的发生率及危险因素,有助于临床中早期识别预防治疗失败高风险的患者,进一步优化抗真菌预防治疗方案。由于不同国家和地区的流行病学、抗真菌预防及治疗的差异,不同研究得出的发生率及危险因素也有所不同。为进一步了解我国HSCT后IFD及bIFD的临床特征及危险因素,本研究回顾性分析了2016-2018年我院233例HSCT受者中确诊和临床诊断的IFD相关资料,报告如下。
1 资料与方法 1.1 一般资料收集本院血液科2016-2018年进行HSCT的患者共233例。其中男性123例(52.8%),女性110例(47.2%)。年龄12~68岁,中位年龄38岁。其中自体造血干细胞移植(autologous hematopoietic stem cell transplantation,auto-HSCT) 111例,异基因造血干细胞移植(allogeneic hematopoietic stem cell transplantation,allo-HSCT) 122例。原发疾病种类包括:急性髓系白血病73例(31.3%),急性淋巴细胞白血病31例(13.3%),慢性粒细胞性白血病5例(2.1%),霍奇金淋巴瘤8例(3.4%),非霍奇金淋巴瘤57例(24.5%),多发性骨髓瘤30例(12.9%),再生障碍性贫血16例(6.9%),骨髓增生异常综合征7例(3.0%),急性混合细胞白血病2例,NK细胞淋巴瘤、淋巴瘤白血病、阵发性睡眠性血红蛋白尿、嗜血细胞综合征各1例。在122例allo-HSCT受者中,有116例来源于亲缘供者,6例为非亲缘供者。人类白细胞抗原(human leukocyte antigen,HLA)配型全相合43例,HLA不全相合79例。移植前有肺部基础疾病者8例。移植前有真菌感染病史者39例,术前31例应用伏立康唑(4 mg/kg,2次/d),3例应用伊曲康唑(200 mg/次,2次/d),4例应用卡泊芬净(50 mg/d),1例应用米卡芬净(50 mg/d)进行再次预防治疗。本研究经本院伦理委员会审批通过(2020-302)。
1.2 预处理方案及GVHD的预防患者均进行移植前预处理,再生障碍性贫血患者采用FC(氟达拉滨+环磷酰胺)方案,多发性骨髓瘤患者采用Mel(马法兰)方案,淋巴瘤患者采用BEAM(卡莫司汀+依托泊苷+阿糖胞苷+马法兰)、CEAC(洛莫司汀+依托泊苷+阿糖胞苷+环磷酰胺)等方案。其余血液病患者有38例采用FLAG+BU/CY方案:氟达拉滨-10~-7 d,30 mg/(m2·d);阿糖胞苷-10~-7 d,2 g/(m2·d);白消安-6~-4 d,3.2 mg/(kg·d);环磷酰胺-3~-2 d,1.8 g/(m2·d)。71例患者采用BU/CY为主的方案:白消安-7~-4 d,3.2 mg/(kg·d);环磷酰胺-3~-2 d,60 mg/(kg·d)。移植物抗宿主病的预防采用环孢素A、甲氨蝶呤、霉酚酸酯、他克莫司。79例HLA不全相合受者加用抗胸腺细胞球蛋白(anti-thymocyte globulin,ATG)预防GVHD。
1.3 感染的预防患者在移植前均接受全面查体,药浴后进入百级层流病房,予以无菌饮食,行口腔、眼、鼻护理,留置深静脉导管。积极收集血液、痰液、尿液、大便、支气管内镜灌洗液等进行细菌及真菌培养。使用喹诺酮类、制霉菌素行肠道细菌、真菌预防,膦甲酸钠、更昔洛韦等防治病毒感染,磺胺甲噁唑预防卡氏肺孢子菌病。如出现感染,予以经验性广谱抗生素静注,根据病原学结果调整治疗。具有IFD高危因素的患者,出现临床感染症状前应用氟康唑、泊沙康唑等预防IFD发生。
1.4 观察及随访中性粒细胞绝对值(absolute neutrophil count,ANC)<0.5×109/L为粒细胞缺乏。移植后粒系重建是指连续3 d以上ANC>0.5×109/L,以第1天为中性粒细胞植活时间。巨核系重建是指不依赖输注血小板的情况下,连续3 d以上血小板>20×109/L。在实际工作中,当患者血小板<20×109/L时本科移植中心会预防性输注血小板,故以外周血ANC>0.5×109/L持续3 d以上为标准。大剂量糖皮质激素应用界定为应用(泼尼松当量)超过2.0 mg/(kg·d)持续1周以上。随访终点为死亡或造血干细胞移植后6个月。
1.5 诊断标准IFD的诊断标准根据中国侵袭性真菌感染工作组制定的《血液病/恶性肿瘤患者侵袭性真菌病的诊断标准与治疗原则(第五次修订版)》[4]。指南中将诊断标准分为:确诊、临床诊断、拟诊和未确定。本研究中将HSCT后确诊及临床诊断的IFD患者纳入病例组。突破性侵袭性真菌病(bIFD)定义为抗真菌预防治疗下发生的确诊/临床诊断的IFD。
1.6 统计学分析采用SPSS 25.0统计软件进行分析。结合临床工作经验,筛选出可能与IFD发生有关的影响指标,以是否发生IFD为结局变量,进行单因素及多因素分析。计数资料组间比较采用χ2检验,若存在单元格期望频数<5的情况使用Fisher精确检验法。单因素分析P < 0.15进入多因素分析,多因素分析采用Logistic回归模型法。P < 0.05表示差异有统计学意义。
2 结果 2.1 IFD的发生情况233例造血干细胞移植患者中有41例诊断为IFD,发生率为17.6%(41/233)。确诊IFD者7例(3.0%),临床诊断IFD者34例(14.6%),其中19例确诊/临床诊断IFD患者在住院期间及出院后6个月内死亡,病死率46.3%。造血干细胞移植前有IFD史者39例,其中移植前抗真菌治疗为完全缓解(complete remission,CR)者17例,部分缓解(partial remission,PR)者18例,稳定(stable disease,SD)者4例,分别有2例、10例、3例在移植后再次发生确诊/临床诊断的IFD,其差异具有统计学意义(χ2=9.685,P=0.005),提示移植前抗真菌治疗完全缓解者在移植后发生IFD的风险更低。移植前有IFD史较无IFD史者术后发生IFD的风险明显增高(38.5% vs 13.4%,OR=4.038,95%CI:1.877~8.687,P < 0.001)。
在41例确诊/临床诊断的IFD中,有32例被认为是bIFD,发生率为13.7%(32/233),其中17例患者在住院期间及出院后6个月内死亡,病死率为51.5%。bIFD与非突破性IFD病死率的差异无统计学意义(OR=3.967,95%CI:0.712~22.108,P=0.140)。
2.2 IFD的临床特征在41例确诊/临床诊断的IFD患者中,急性髓系白血病(acute myeloid leukemia,AML)是最常见的原发病,占36.6%(15/41),其次为再生障碍性贫血(aplastic anemia,AA),占17.1%(7/41)。8例(19.5%)被诊断为侵袭性念珠菌病,33例(80.5%)被诊断为侵袭性霉菌病。共检出11株病原菌,其中热带假丝酵母菌6株,近平滑念珠菌2株,光滑假丝酵母菌1株,烟曲霉1株,黑曲霉1株。感染部位:血液7例(其中1例合并消化道),下呼吸道34例。在IFD患者中,临床症状及体征均缺乏特异性。真菌血症患者主要以高热、畏寒、寒战为主要表现,且广谱抗生素抗感染治疗无效。肺部真菌感染的患者主要表现为发热、咳嗽、咳痰、呼吸困难,双肺散在湿罗音等表现。部分患者肺部影像学可表现为伴有“晕征”的结节、实变影、新月征、空洞等,见图 1。
2.3 IFD的危险因素分析
单因素分析显示异体移植、移植前IFD史、CMV感染、EB病毒感染、肺部基础疾病、粒细胞缺乏>14 d、大剂量激素使用是IFD发生的危险因素,而年龄、性别、移植季度、原发疾病为AML/MDS、移植前疾病状态、化疗周期数、糖尿病与IFD的关系无统计学意义。Logistic回归分析显示移植前IFD史(OR=5.379,95%CI:2.146~13.480;P<0.001)、EB病毒感染(OR=3.333,95%CI:1.141~9.735;P=0.028)、肺部基础疾病(OR=10.786,95%CI:2.162~53.800;P=0.004)、粒细胞缺乏>14 d(OR=6.759,95%CI:2.931~15.583;P < 0.001)、大剂量激素的使用(OR=3.009,95%CI:1.124~8.052;P=0.028)为IFD的危险因素。具体见表 1。

在122例allo-HSCT受者中,单因素分析显示供受者亲缘关系、HLA相合情况、ATG的使用、急慢性GVHD与IFD的关系无统计学意义。Logistic回归分析显示慢性GVHD(OR=14.143,95%CI:3.049~65.614;P=0.001)为IFD的独立危险因素。具体见表 2。

以确诊/临床诊断的IFD中突破性感染者为病例组,非突破性感染者为对照组,单因素分析提示粒细胞缺乏>14 d为bIFD的危险因素,而与性别、年龄、移植方式、移植季度、移植前疾病状态、化疗周期数、移植前IFD史、CMV感染、EB病毒感染、糖尿病、肺部基础疾病、大剂量激素使用之间的关系无统计学意义。Logistic回归分析提示粒细胞缺乏>14 d(OR=12.000,95%CI:1.248~115.362;P=0.031)为bIFD的危险因素,具体见表 3。

3 讨论
侵袭性真菌病(IFD)是导致造血干细胞移植(HSCT)患者死亡的重要原因之一。国内外多项研究显示HSCT后IFD的发病率不断上升[6-7]。本研究显示,HSCT后确诊/临床诊断的IFD的发生率为17.6%,病死率46.3%。国内外多个单中心研究报道的IFD发生率在13.6%~23.0%,病死率50.0%~65.7%[8-11],均与本研究结果相当。而中国血液病抗真菌治疗评估(China Assessment of Antifungal Therapy in Hematological Disease,CAESAR)中IFD的发生率(7.7%)及病死率(18.6%)低于各个单中心数据[12]。这可能由各中心患者的疾病构成、临床特征、诊断方式及防治方案的差异造成。这项研究还显示allo-HSCT受者的IFD累积发病率明显高于auto-HSCT患者(P < 0.001),与本研究结论相一致。
近年来,念珠菌引起的真菌血症发病率逐渐下降,而侵袭性霉菌病成为IFD的主要组成部分。同时,念珠菌的菌种由白色念珠菌向非白色念珠菌转变,烟曲霉以外的曲霉及非曲霉菌如毛霉菌、镰刀菌引起的IFD发生率逐渐升高[13]。本研究中IC的比例明显低于IMD(19.5% vs 80.5%),明确霉菌种类的2例中就有1例为黑曲霉,且IC的病原菌均为非白色念珠菌,以热带念珠菌为主,与既往研究报道的以白色念珠菌为主不一致[13],考虑本中心病原菌分布与其他地区存在差异,但也不排除样本量较少缺乏一定的代表性。
IFD预防策略的制定不仅要考虑当地病原菌谱的构成,还应对主要危险因素进行分析,依据危险分层做到精准预防。国内一项对CEASAR研究的分析指出,低危患者在预防治疗中并未获益,而中高危患者的IFD发生率较未进行预防治疗组明显降低[14]。
本研究得出cGVHD是IFD的独立危险因素,既往研究表明急慢性GVHD与IFD风险增加有关[10, 5]。CHIEN等[16]分析发现IMD患者发生中到重度cGVHD明显高于对照组(P=0.001)。近期还有研究提出IFD对allo-HSCT后cGVHD的发生也有促进作用(P < 0.001)[17]。GVHD的发生可导致皮肤黏膜防御屏障的损伤,其次防治急慢性GVHD中免疫抑制剂的应用可增加霉菌及酵母菌感染风险,并增强这些病原体的毒力[15]。因此,导致合并GVHD的患者IFD发生率增高。
已有研究发现既往合并肺部疾病(如肺气肿、空洞性肺结核)与慢性坏死性肺曲霉菌病的发生有关[18]。CHOI等[11]也得出肺部基础疾病是早期IFD的独立危险因素,并认为IMD可能来源于吸入的真菌孢子或肺部真菌的定植,而气道基础疾病和肺部解剖缺陷导致黏膜纤毛功能受损,为真菌孢子定植提供了良好的环境。本研究也发现肺部基础疾病是IFD的独立危险因素,也更加印证了这一结论。然而在本研究中具有肺部基础疾病的病例数较少,可能在一定程度上影响了结果的可靠性。
目前对于ATG的使用是否增加IFD的发生风险存在争议。LI等[19]发现ATG可增加IFD的发生率(P=0.002),但ROBIN等[10]指出未使用ATG增加晚期IA的发生率(P=0.06)。更多的研究并未得出ATG的使用与IFD的发生有关[20, 21],本研究也是如此。BARON等[22]认为ATG输注诱导淋巴细胞凋亡,产生强免疫抑制作用,从而增加真菌感染风险,但同时ATG可诱导调节性T细胞的产生,在GVHD的预防中起到重要作用,降低IFD的发生率。
气象条件和环境温度的季节性变化会影响病原体的毒力、宿主的行为以及易感性,造成真菌流行病的地方性。PANACKAL等[23]发现夏季月份接受HSCT增加IA发生的风险,IA的发生率与环境孢子数呈正相关,而高温和低降水量都会增加环境中的孢子数。我们试图分析移植季度是否对IFD产生影响,但未得出阳性的结果,未来有望进一步引入温度、湿度等具体因素来评估环境对IFD的影响。
过去的十年中,抗真菌预防治疗的应用降低了HSCT受者的IFD发生率和死亡率[19, 24-25],但突破性感染仍会发生[26-28]。一项奥地利的单中心研究显示,泊沙康唑预防治疗下bIFD的发生率为13%,且感染毛霉菌和其他罕见真菌病原体的比率增加[29]。本研究中突破性感染的发生率与既往研究相当,但在实际工作中突破性与非突破性IFD的区分十分困难,且国内外bIFD的定义尚未统一,也影响本结果的横向比较。2018年意大利专家组将突破性IFD定义为在抗真菌初级预防开始7 d后和结束后7 d内诊断的确诊/临床诊断IFD[26]。不久前美国真菌病研究组教育和研究联合会以及欧洲真菌学医学联合会将其定义为暴露于抗真菌药物期间发生的任何侵袭性真菌感染[30]。
抗真菌预防治疗下IFD的突破是由多方面的因素造成的。本研究得出粒细胞缺乏>14 d是bIFD的危险因素。而既往研究还发现bIFD的危险因素有严重的基础疾病、HLA不全相合、全身性使用糖皮质激素、混合感染等[25, 28]。对于突破性念珠菌血症,严重的黏膜炎、中心静脉导管、血药浓度低也是其危险因素31, 32]。
多个研究分析指出,不同的抗真菌药物预防下bIFD的发生率及死亡率无明显差异[5, 33-34],但也有研究表明泊沙康唑可降低bIFD的发生率和死亡率[35]。此外,由于念珠菌极易在各种血管导管中定植,对于突破性念珠菌血症的患者建议及时拔除导管[31]。多个研究及指南还建议进行定期的治疗药物监测以达到最佳预防治疗效果[36]。由于本研究中样本量有限,并且bIFD的病例数较少,关于突破性侵袭性真菌病的流行病学、危险因素以及治疗策略还需要进行更多大规模回顾性、前瞻性研究。
对于IFD危险因素的分析具有重要意义,有助于我们早期识别高危患者,及时进行有效的抗真菌治疗。目前世界各国研究者们仍在不断探索IFD的发生机制、宿主的遗传学特征,寻求更加敏感特异的诊断方式以及安全高效的治疗药物,多学科的联合有望进一步降低IFD的发生率及死亡率,从而改善患者预后。
