[Abstract]: Human leukocyte antigen (HLA)-haploidentical hematopoietic stem cell transplantation (HLA-haplo HSCT) without T-cell depletion has tremendously progressed over the past 20 years. Advances in graft manipulation, conditioning regimens, and pharmacological graft-versus-host disease (GVHD) prophylaxis have reduced the risk of fatal graft failure and severe GVHD. Donor-specific anti-HLA antibodies (DSA) negative status and killer immunoglobulin-like receptor (KIR) mismatch play potential roles in reducing the risk of graft failure and GVHD following HLA-haplo HSCT. New strategies to improve HLA-haplo HSCT outcomes include donor lymphocyte, selected T-cell subset and NK cell infusion, mesenchymal stem cell (MSC) co-transplantation and interleukin-2 application. Future challenges remain in improving post-transplant immune reconstitution and finding the best approach to reduce the incidence and severity of GVHD while simultaneously preserving the graft-versus leukemia effect to prevent the recurrence of underlying malignancy.

HLA单倍型相合造血干细胞移植(human leukocyte antigen-haploidentical hematopoietic stem cell transplantation,HLA-haplo HSCT)是大多数血液病患者可选择的一种移植方式,能够节约寻找供者的时间和花费[1-2]。二十多年以来,多种单倍型相合移植方式,包括保留或去除T细胞的单倍型移植显示出了较好的预后[3-4]。多个移植中心报道,去除T细胞的外周血干细胞移植发生移植物抗宿主病(graft-versus-host disease,GVHD)的概率较低,但是,由于免疫功能恢复 的延迟,该移植方式有较高的感染和肿瘤复发风险,增加了移植后死亡率,尤其是CD34+细胞纯化移植[2, 5-6]。 因此,许多中心尝试采用不去除T细胞的骨髓干细胞方案[2, 4, 7],使用抗胸腺细胞免疫球蛋白(antithymocyte globulin,ATG)达到在体内去除T细胞的目的;用粒细胞集落刺激因子(granulocyte colony stimulating factor,G-CSF)动员造血干细胞,以期达到诱导T淋巴细胞向Th2方向分化的目的;或者移植后大剂量环磷酰胺选择性杀灭异体激活T细胞[7-8]。我们将从预处理方案、GVHD预防、移植后真菌和巨细胞病毒感染、提高移植预后的方法等方面作一评述,并展望这一移植技术的未来。

1 预处理方案 1.1 清髓性预处理造血干细胞移植

北京大学研究了采用G-CSF动员后的骨髓和外周血干细胞联合移植,并使用ATG体内去除T细胞的方案;预处理药物包括阿糖胞苷、白消胺、环磷酰胺、司莫斯丁,GVHD预防采用环孢素+马替麦考酚酯+短程甲氨蝶呤[8]。在他们的首次报道中,比较了单倍型相合造血干细胞移植和不使用ATG的同胞间全相合造血干细胞移植的预后,其中Ⅱ到Ⅳ度急性GVHD的累积发生率分别为40%和32%(P=0.13);令人惊讶的是,两者的移植相关死亡率(22%和14%)、复发率(18%和13%)、总体生存率(71%和72%)都非常接近。其后,他们进一步总结了未去除T细胞的单倍型相合造血干细胞移植治疗145例Ph+急性淋巴细胞白血病(Ph+acute lymphoblastic leukemia,ALL)和450例急性髓系白血病(acute myeloid leukemia,AML)患者,预处理方案如下:全身放疗(total body irradiation,TBI)+ 洛莫司汀+阿糖胞苷+环磷酰胺+ATG,或白消胺+洛莫司汀+阿糖胞苷+环磷酰胺+ATG。AML患者 的3年预期无白血病生存率为74%,Ph+ ALL患者5年 预期无白血病生存率为65.8%[6, 9]。以上结果提示未去除T细胞的单倍型相合造血干细胞移植与同胞间HLA全相合移植预后相当。

一项来自中国西部地区的多中心随机对照研究比较了G-CSF预激的预处理方案对非去除T细胞的单倍型相合造血干细胞移植治疗高危AML预后的影响。G-CSF剂量为5 mg/(kg·d),-10 d~-7 d皮下注射;尼莫司汀200 mg/m2,-9 d顿服;大剂量阿糖胞苷4 g/(m2·d),-8 d~-7 d;白消胺3.2 mg/(kg·d),-6 d~-4 d;环磷酰胺1.8 g/(m2·d),-3 d~-2 d[10]。G-CSF可以使静止期的白血病干细胞进入增殖周期、诱导T细胞向Th2方向分化,故预处理中G-CSF和大剂量阿糖胞苷联合可以增强对白血病细胞的杀伤作用,并能减轻GVHD、减少白血病复发[11-12]

1.2 非清髓性预处理造血干细胞移植

虽然清髓性预处理能够减少植入失败和GVHD发生的概率,但也伴随着高预处理相关毒性、感染发生率及移植相关死亡率,因而限制了其在单倍型相合造血干细胞移植的应用范围。于是,多个移植中心均尝试了非清髓性预处理方案,得到了较好的结果[13]

多数非清髓性预处理方案包含一种强效免疫抑制药物——氟达拉滨。德国杜宾根大学、美国杜克大学研究采用氟达拉滨联合OKT3[14]或者阿仑单抗[15]体内去除T细胞的预处理方案,这些方案血液学毒性小,同时又能保证供者造血干细胞植入,移植后1年总 生存期为31%~37%[14-15],移植年龄最大可达66岁,拓宽了单倍型相合造血干细胞移植的适用范围。

近期,日本的一项由5家中心参加的前瞻 性Ⅰ/Ⅱ期临床试验观察了低剂量ATG联合糖皮质激素用于非清髓性单倍型相合造血干细胞移植预处理[16],一共纳入34例处于疾病进展期或有高复发风险血液肿瘤患者。预处理方案为:氟达拉滨+白消胺+ATG(费森尤斯,8 mg/kg),GVHD预防采用他克莫司+甲氨蝶呤(1 mg/kg)。33例患者供者细胞植入成功,Ⅱ~Ⅳ度急性GVHD和广泛型慢性GVHD累积发生率分别为30.7%和20%;14例患者复发(42.2%);移植后1年累积移植相关死亡率(transplantation related mortality,TRM)为26.5%。1年生存率,完全缓解/慢性期(8例)患者为62.5%,未达完全缓解患者(26例)为42.3%。结果表明,这一移植方案对于那些短时间内无全相合供者的患者来说是安全可行的。

2 GVHD的预防 2.1 G-CSF动员后的骨髓+外周血干细胞联合移植

G-CSF能够通过增加浆细胞样树突状细胞和抑制CD28-CD80/86信号通路,诱导T细胞向Th2细胞转化[17]。基于以上研究结果,中国科学家黄晓军等提出了一种新的非清髓单倍型相合移植方案,加入ATG以 增强免疫抑制,移植物采用G-CSF动员后的供者骨髓+ 外周血干细胞[7, 10, 17]。他们最新报道了450例急性白血病患者,231例(51.3%)采用了该移植方案[18]。供者采用G-CSF 5 mg/(kg·d)动员造血干细胞,动员第4天采集骨髓,第5天采集外周血干细胞。GVHD预防方案为:环孢素+马替麦考酚酯+ATG+甲氨蝶呤。Ⅱ~Ⅳ度和Ⅲ~Ⅳ度急性GVHD累积发生率分别为36%、10%。1年慢性GVHD累积发生率为42%,3年无病生存率(disease-free survival,DFS)为74%,3年总体生存(overall survival,OS)为79%[18]

2.2 短程他克莫司

钙调节蛋白抑制剂他克莫司在体外对T细胞的抑制强度是环孢素的100倍,以往单独或联合其它免疫抑制剂用于HLA全相合造血干细胞移植的GVHD预防[19]。低剂量他克莫司可以诱导调节性T细胞[20];他克莫司和马替麦考酚酯都有抑制Th1相关基因转录、维持Treg/Th2表型的作用[21]。我们以往的单臂回顾性研究证实,采用他克莫司预防单倍型相合造血干细胞移植患者GVHD是可行的;但是,长疗程使用他克莫司可能增加感染特别是巨细胞病毒感染的发生率[22]。基于前期研究结果,我们比较了短疗程他克莫司+甲氨蝶呤+马替麦考酚酯和经典的环孢素+甲氨蝶呤+马替麦考酚酯方案预防单倍型相合造血干细胞移植患者GVHD的疗效和安全性。移植后100 d累积Ⅲ度急性GVHD发生率分别为29.1%和50.0%(P=0.005),Ⅳ度急性GVHD发生率分别为3.6%和13.5%(P=0.0027)。两组在慢性GVHD、移植后复发、巨细胞病毒(cytomegalovirus,CMV)感染、无病生存和总体生存上没有明显差别。淋巴细胞亚群分析显示,短程他克莫司组患者T淋巴细胞在移植后3个月内有小幅度下降。综上,短程他克莫司能够降低单倍型相合移植急性GVHD发生率和严重程度,同时不增加复发和CMV感染。

2.3 移植后环磷酰胺

因为具有廉价、高效且操作简单的特点,移植后使用环磷酰胺成为了一项非常有吸引力的GVHD预防措施。移植后环磷酰胺诱导双向免疫耐受有多种可能的机制:①选择性杀伤抗原激活T细胞一系列的证据证实幼稚T细胞和效应性/记忆性T细胞对环磷酰胺介导的杀伤作用敏感性不同。小鼠实验表明,效应性/记忆性T细胞对环磷酰胺的杀伤有相对较强的耐受性,这一特性或许有利于长期外周血T细胞池和免疫功能恢复[23]。 ②环磷酰胺可以清除胸腺中供者来源的抗宿主T细胞。这一机制不受TOR样受体的影响,对终生免疫耐受的维持至关重要[23]。③晚期克隆清除和调节性/抑制性T细胞的产生。CD4+调节性T细胞可能对环磷酰胺介导的移植后免疫耐受至关重要——这一观念与近期的一项研究结果一致[24]

基于上述临床前研究结果,许多中心开始了单倍型相合移植后环磷酰胺预防GVHD的临床研究。Luznik等[25]采用减低剂量预处理,在移植+3、+4 d 给予患者100 mg/kg的环磷酰胺。移植后200 d 内Ⅱ~Ⅳ度和Ⅲ~Ⅳ度急性GVHD累积发生率分别为34%和6%;广泛型慢性GVHD累积发生率为5%;2年总生存为36%,无事件生存率为26%。除了最初用于降低剂量预处理的移植外,近期几项小样本的临床研究尝试将环磷酰胺用于非清髓的外周血干细胞移植。Bacigalupo等[26]报道了148例单倍型相合移植的血液肿瘤患者,所有患者采用非清髓预处理方案:塞替派+Bu+Flu或者TBI+Flu;移植后+3、+5 d给予环磷酰胺,0 d起给予环孢素,+1 d起给予马替麦考酚酯预防GVHD。Ⅱ~Ⅳ度急性GVHD累积发生率为24%,Ⅲ~Ⅳ急性GVHD发生率为10%;中到重度慢性GVHD发生率为12%。22个月的总体生存率CR1患者为77%,CR2患者为38%,而复发患者为38%。此项研究表明,非清髓单倍型相合造血干细胞移植后给予环磷酰胺治疗,急性、慢性GVHD发生率较低,移植相关死亡和总体生存均较满意。

3 移植后侵袭性真菌病和CMV感染 3.1 移植后侵袭性真菌病

由于免疫功能重建缓慢,侵袭性真菌病(invasive fungal disease,IFD)成为体外去T单倍型相合造血干细胞移植患者移植相关死亡和感染相关死亡的重要因素[27]。Perugia大学一项样本量为205例的临床研究表明,体外去除T细胞的单倍型相合移植后侵袭性曲霉菌病发生率比HLA全相合移植高2.7倍[28]。非体外去除T细胞的单倍型相合移植采用包含ATG的预处理方案实现体内部分去除T细胞;G-CSF动员后的移植物可诱导T细胞向Th2分化;移植后使用雷帕霉素可促进调节性T细胞亚群恢复;移植后大剂量环磷酰胺可以选择性去除同种反应性T细胞[6, 22, 25-26]。一项单中心的头对头临床研究[29]评估上述治疗措施是否有助于降低侵袭性真菌病的发生。在1 042例患 者中,有390例接受HLA全相合造血干细胞移植,652例 接受非去除T细胞的单倍型相合造血干细胞移植;IFD的评价标准参照欧洲癌症治疗研究组织/美国国家过敏症与传染病研究所霉菌病研究组(EORTC/MSG)标准,将确诊和拟诊的患者纳入统计。共有61例(5.8%)患者发生IFD,包括15例确诊和46例拟诊。单倍型相合移植患者的IFD发生率较全相合患者稍高,分别为7.1%、3.3%(P=0.007);发生时间单倍 型相合移植患者早于全相合患者,分别为移植后23.0、 141.5 d(P=0.04)。在多因素分析中,Ⅲ~Ⅴ度急性GVHD发生率、广泛型慢性GVHD发生率、单倍型相合移植是IFD发生的危险因素。IFD的预后与移植类型无关。上述结果表明,我们需要采取更多积极的措施来预防单倍型相合移植患者的IFD。

3.2 移植后CMV感染

尽管采用了相应的预防和抢先治疗措施,CMV感染仍是影响单倍型相合造血干细胞移植预后的重要因素[30]。来自北京大学的研究者建立了GIAC单倍型相合造血干细胞移植体系,他们发现,采用该体系移植的患者移植100 d后CMV感染率高于全相合移植患者(65% vs 39%),而CMV肺炎的发生率两组相似(均为17%)[31]。Kurokawa等[32]日本学者进行了66例成年血液肿瘤患者非去除T细胞的单倍型相合造血干细胞移植,所有患者采用减低剂量预处理,在57例可统计患者中,有45例发生了CMV血症,中位发生时间为移植后19 d;3例诊断为CMV相关性疾病,1例因CMV结肠炎死亡。

免疫细胞亚群重建可能是影响移植预后最重要的因素。在免疫功能正常的CMV血清学阳性个体,其血清中能够检测到大量的CD4+、CD8+CMV特异性T细胞,这些细胞可以抑制CMV病毒的激活[33]。移植后CMV特异性T细胞数量和活性的恢复对CMV感染的控制至关重要。目前临床上可以通过检测CMV特异性T细胞功能来决定开始抗CMV治疗,这一技术能够减少抗病毒治疗的支出。移植后免疫重建受多重因素的影响,包括供患者年龄、基础疾病、移植类型、预处理方案、干细胞来源、HLA相合程度、GVHD、感染等[34]。近期一项研究提示年轻供者、外周血来源干细胞、抗病毒治疗时亚临床的CMV再激活、无GVHD和减少糖 皮质激素的使用有利于抗CMV特异性免疫的恢复[35]

4 改善移植预后的方法 4.1 供者选择

多数患者能找到1个以上的潜在单倍型相合供者,如何选择最合适的供者?在众多选择因素中,杀伤细胞免疫球蛋白样受体(killer immunoglobulin-like receptor,KIR)错配和供者特异性抗HLA抗体(donor-specific anti-HLA antibodies,DSA)是两个主要因素。

多个临床研究发现供受者KIR错配与较好的移植预后相关[36-37]。Ruggeri等[36]观察发现,AML患者如果选择KIR错配供者,有利于减少植入失败、GVHD和疾病复发。Symons等[37]也报道了类似的研究结果,在一项86例血液肿瘤患者参与的队列研究中,所有患者接受了非清髓预处理和移植后环磷酰胺治疗,其中选择KIR错配供者的患者在非复发死亡率、总体生存和无事件生存率上优于非错配患者。T细胞同种异体反应在单倍型相合移植中占据重要地位,我们需要进一步研究KIR错配在供者选择中的意义和机制,以期改善移植预后。

因为DSA阳性可能增加植入失败风险,故DSA阳性是供者选择的绝对禁忌。目前有3种方法检测DSA:①淋巴细胞毒3交叉配型,②流式细胞交叉配型,③基于HLA单分子的荧光免疫磁珠的固态免疫测定法(solid-phase immunoassay,SPI);其中SPI是目前敏感性最高的DSA测定方法[38]。Cruiea等[39]分析 了122例单倍型相合移植受者的C1q结合DSA(C1q+DSA)。结 果有22例受者C1q+DSA为阳性,其中19例为女性,7例植入失败。植入失败病例移植时中位 DSA浓度为10 055 MFI(median fluorescence intensity),而植入成功病例DSA中位浓度仅为2 065 MFI(P=0.007)。该试验表明,患者具有高DSA水平(>5 000 MFI)是植入失败的高度危险因素;C1q+DSA检测应 该作为异基因移植前的必要检测项目,同时,降低C1q+ DSA水平也可能提高植入成功率。

4.2 供者淋巴细胞输注

单倍型相合移植后供者淋巴细胞输注(donor lymphocyte infusion,DLI)的临床研究较少。Lewalle等[40]提出,单倍型相合移植后DLI起始输注的细胞量应为105/kg。在一项以色列的研究中,有28例患者接受了预防性(n=6)或治疗性(n=22)DLI,输注细胞数量从1×102~1.5×109/kg不等[41];在22例患者中有6例 治疗有效,肿瘤负荷较高的患者疗效较差。Wang等[42] 采用G-CSF动员后的DLI预防肿瘤复发,他们共观察了88例移植后急性白血病进展的患者,其中61例进行G-CSF动员后DLI,27例未使用DLI。采用了预防性DLI的患者2年复发率和未采用DLI的患者分别为36%和55%(P=0.017);3年预期OS分别为31%和11%(P=0.001),无事件生存率为22%和11%(P=0.003)。在多因素分析中,移植后预防性DLI是一项独立的预后良好因素。作者随后回顾性分析了单纯化疗、化疗+G-CSF动员DLI对单倍型移植后复发患者的疗效。结果移植后DLI+化疗患者的完全缓解率明显高于单纯化疗患者(64.0% vs 12.5%,P=0.000),复发率明显降低(50.0% vs 100.0%,P=0.000),无病生存期有较大提高(36.0% vs 0.0%,P=0.000)[43]。Zhou等[44]长期随访了10例HLA单倍型相合移植后输注半胱氨酸蛋白酶9修饰的T细胞(iC9-T)患者,输注后患者立刻获得了持久性的对CMV、腺病毒、BK病毒以及EB病毒免疫力,但是并没有出现急性或慢性GVHD,提示单倍型相合移植后淋巴细胞输注对免疫功能恢复具有积极的效果。

4.3 供者NK细胞输注

NK细胞的同种反应性可改善HLA单倍体造血干细胞移植的疗效及安全性。NK细胞被认为可通过其抑制和激活受体识别他们的靶点。Duke大学研究中心对接受了51次NK细胞输注的造血干细胞移植的患者(其中包含14例相合及16例不相合)分析时发现,接受NK细胞输注且具有持续反应的患者具有更好的预后[45]。基于这一惊喜的结果,许多研究者评估了HLA单倍体造血干细胞移植术后NK细胞输注的可行性,并利用其自身免疫性的特点针对不同肿瘤进行了研究。近年来,NK细胞纯化技术的成功为NK细胞输注作为HLA单倍型造血干细胞移植术后的肿瘤的预防或复发提供了实验基础[46]。Yoon等[47]报道了14例接受了HLA单倍型造血干细胞移植的急性髓系白血病及MDS患者,术后接受了6~7周的造血干细胞来源的NK细胞输注,研究结果表明所有患者皆未出现输注相关的副作用,4例患者发生了cGVHD,4例患者移植术后的无病生存期可达18~20个月,只有2例患者接受NK细胞输注后无疗效。Choi等[48]对41例接受了减低剂量的造血干细胞移植的恶性血液病患者研究时发现,所有患者在移植术后2~3周接受两次NK细胞输注,剂量按(2~10)×107个/kg梯度进行输注。研究结果发现,所有剂量组皆未出现毒副作用,并且与同时期接受移植但未接受NK细胞输注的患者相比较,移植后相关事件(如植入、Ⅱ~Ⅳ的aGVHD、中度及重度的cGVHD及TRM)皆无统计学差异。然而,移植术后接受NK细胞输注的患者肿瘤进展的比例(46%)较未接受NK细胞输注的患者(74%)明显减少,同时预后危险因素回归分析时发现,移植后接受NK细胞输注是减少白血病进展的独立预后因子(风险比=0.527)。为了进一步明确NK细胞输注的治疗效果,仍然需要更多的前瞻性研究进行验证。

4.4 T细胞亚群输注

未经修饰的供者T细胞输注可作为一种理想的替代治疗手段,许多研究者对以下2种方式的可行性作了测试: ①输注已经清除自身反应性T细胞亚群的供者T细胞[49]; ②将T细胞转导单纯疱疹病毒胸苷激酶自杀基因载体,此种T细胞在发生严重GVHD的情况下能被清除[50]。Amrolia等[51]采用CD25单抗清除具有自身免疫活性的淋巴细胞及去除自身免疫活性的T细胞的干细胞液输注入受者体内后,接受大剂量T细胞输注的患者约4/6可观察到病毒特异性反应,并且所有患者严重GVHD的发生率皆较低。近年来有研究表明,白血病细胞可以改变其自身的HLA表达,从而逃避供者T细胞的杀伤作用,这可能是肿瘤复发的另一重要原因,但具体机制有待进一步探讨[52]。因此在进行供者T细胞输注时,复发的白血病细胞的HLA位点有必要进行重新检测。Di-Ianni等[53]作了如下研究,28例高风险的恶性血液病患者分别接受清髓性的预处理方案后再输注2×106/kg剂量的Treg细胞,4 d后,所有患者再接受1×106 Tcons细胞及10×106CD34+细胞,研究人员发现在未给予免疫抑制的治疗下,所有患者发生GVHD的风险都较低,有趣的是,移植后的免疫重建的方式与标准的去除T细胞的造血干细胞移植不同,此种移植方式患者T细胞亚群重建较快,种类较均衡,同时抗原特异性的CD4+及CD8+T细胞比例较高,值得注意的是CMV病毒的检出率及CMV相关性死亡皆较低。这种创新的移植方式可确保此后成熟T细胞的输注,有利于早期免疫系统的重建和改善患者的预后。

4.5 骨髓间充质干细胞

大量研究表明骨髓间充质干细胞在体内和体外均具有免疫调节功能[54]。骨髓间充质干细胞在体外以剂量依赖性和时间限制的方式调节T、B淋巴细胞的增殖,活化和成熟[55]。成年HLA同胞间相合移植患者,输注骨髓间充质干细胞是安全的,它可能加速造血功能的恢复和减少急性及慢性GVHD的发生率。Lazarus等[56]已经证实骨髓间充质干细胞与造血干细胞共移植是可行的、安全的,没有出现速发以及迟发的输注不良反应。和HLA单倍型造血干细胞移植相比,在接受骨髓间充质干细胞输注的持续供者植入的患者中观察到的排斥反应的风险较小。Ball等[57]在14例接受去T细胞单倍体移植的儿童患者中,采用了共移植供体来源的骨髓间充质干细胞的方法。这些患者中没有一例出现了不良反应以及移植失败。此外,Zhou等[58]以及Zhao等[59]同时提出输注体外扩增的骨髓间充质干细胞,对于治疗激素抵抗的慢性GVHD是安全、有效的。我们中心的研究评估的是HLA单倍型移植患者间断输注骨髓间充质干细胞,预防慢性GVHD的疗效及安全性。我们发现这些患者的慢性GVHD以及严重的肺部慢性GVHD的2年累积发生率降低。输注骨髓间充质干细胞后,NK细胞数量下降,但是记忆B淋巴细胞的数量和Th1:Th2比例增高。

4.6 白介素-2

白介素-2是一种多功能细胞因子,在免疫反应中起着重要的作用。在造血干细胞移植后出现微小残留病时,早期使用白介素-2,可以降低患者复发率,提高患者的免疫功能[59]。这种效应的产生可能是由于原始CD34+CD105+细胞的淋巴定位(lymphoid orientation),表达高亲和力的白介素-2受体。因此,外源性的IL-2可能导致GVL效应的增强[60]。Liu等[61]的临床研究中,观察了19例成人急性淋巴系统恶性肿瘤,其中包括了6例接受了半相合移植的患者,这6例患者在移植后有高复发的概率,在移植后均接受了白介素-2的治疗。中位随访时间为6(3~19)个月,15例可评价的患者中,14例患者的无病生存率为93.33%,而有1例在移植被评定为高危的患者复发。IL-2的毒性反应包括发热、注射部位的疼痛、发红以及肿胀。我们认为皮下注射低剂量IL-2长达100 d甚至更长,对于预防异基因造血干细胞移植后有高复发风险的成人急性淋巴系统恶性肿瘤的复发,是一种安全、有效的方法。

5 未来方向

过去几年,HLA单倍型移植已经被越来越多的全球移植中心所采用[3, 10, 16, 62]。HLA单倍型移植为可以从移植中获益但是又没有全相合供者的患者提供了一个机会。HLA单倍型移植的最终目的是成功克服HLA屏障,获得中度GVHD所带来的最优的GVL效应。未来有几个新的方法是很有前途的: ①选择性的但是有效的异体清除(allo-depletion),有利于供体成功植入和改进移植后的免疫重建,同时降低GVHD发生率; ②改进DLI,产生不伴有GVHD或者伴有有限 GVHD的GVL效果; 免疫细胞疗法,例如调节性T细胞、NK细胞、T细胞,骨髓间充质干细胞和供体NK细胞以及第三方细胞输注; ③病原体或白血病特异性供体来源的细胞输注,这可能是HLA单倍型造血干细胞移植后预防机会性感染和减少白血病复发率的新方法。
